Job Purpose Under general supervision, plans, coordinates, and provides professional public health nursing assessment and person-centered care planning to individuals, families, and groups in home and community-based settings.
Essential Job Duties- Provides nurse monitoring for the In-Home Aide Services Program
- Provides nursing assessments, person-centered care planning and referral services to individuals, families, groups, and institutions.
- Reviews Maryland Department of Health Long Term Services and Supports Tracking System (LTSS) assessment requests upon assignment of cases.
- Contacts assigned clients/representatives timely.
- Conducts home visits.
- Completes nursing assessments and high-quality person-centered care planning timely to ensure timely access to home and community-based services.
- Provides case management, ongoing monitoring and evaluation of person-centered care plan.
- Assesses and monitors clients' physical and mental states of health and determines need for community-based resources.
- Ensures compliance with state, federal and Medicaid regulations.
- Provides health screening education and counseling in the areas of disease prevention, health maintenance, and mental health.
- Participates in the instruction and orientation of new public health nurses.
- Prepares and maintains nursing records and reports.

Required QualificationsGraduation from an accredited college or university with a bachelor's degree in nursing.
OR Graduation from an accredited diploma school of nursing;
PlusAt least one year of professional nursing experience.
ORGraduation from an accredited college or university with an associate in arts degree in nursing:
Plus at least two years' professional nursing experience.
Additional qualifying education may be substituted on a year-for-year basis for the required experience.
Preferred ExperienceExperience working with a Maternal and Child Health Program and/or Infants and Toddlers Program
Experience working with families of young children with special needs
Experience providing early childhood intervention and preschool special education system of servicesLicenses and CertificatesPossession of a valid license to practice as a professional registered nurse, issued by the Maryland State Board of Examiners of Nurses, is required at the time of appointment. Possession of a valid driver's license equivalent to a noncommercial, class C Maryland driver's license is required at the time of appointment.
