Overview of the Psychiatric Nurse Practitioner role
Wayspring is looking for a full time Psychiatric Nurse Practitioner to conduct visits in a combination of ways including virtually or in our outpatient clinic. This is a hybrid role, with 2-3 days a week in our outpatient clinic, and 2-3 days a week working telehealth from home.
The Psychiatric Nurse Practitioner’s primary responsibility is to facilitate the optimal delivery of care for patients enrolled in Wayspring’s Substance Use Disorder Home solution. This role will be an essential aspect of the program, to promote and execute safe and effective recovery plans for patients’ needs. The Psychiatric Nurse Practitioner will be responsible for screening assessments of new members referred to Wayspring for support when there are mental health conditions that may impact the ability of the patient to successfully engage and achieve program milestones. The Psychiatric Nurse Practitioner will be part of a multi-disciplinary care team consisting of Primary Care Nurse Practitioners, Clinical Care Navigators and Therapists. The Psychiatric Nurse Practitioners will be responsible for reviewing cases for patients who have been escalated for clinical complexity, medication adherence, and care coordination with other providers or care management programs. The Psychiatric Nurse Practitioner will function as the behavioral health clinical expert on the team during rounds, health plan client-facing meetings, or other areas where nursing support is needed in the program delivery.
Responsibilities of the Psychiatric Nurse Practitioner
- Provides direct patient care for Wayspring patients and care collaboration for complex patients with multiple needs, determine the best strategy and support for the patient to encourage engagement (Clinical Care Navigators, Primary Care NPs and Therapists)
- Performs comprehensive health care assessments and treatment plans, including history, MAT services, and physicals within the outpatient clinic to identify substance use disorder and medical needs. On occasion, some of these assessments are done virtually through a telehealth visit using simultaneous video and audio communications.
- Providers substance use disorder treatment plans, including MAT where indicated.
- Drives positive clinical outcomes for Wayspring patients through direct patient care and as an active member of Wayspring’s interdisciplinary care team.
- Completes intakes, attends, and actively participates in collaboration meetings, educational activities, internal rounds discussions, health plan correspondence, pharmacy correspondence, referral partner management, ordering labs and member goal tracking.
- Serves as the behavioral health clinical expert on the team during rounds, health plan client-facing meetings, or other areas where provider support is needed in the program delivery.
- Reviews the initial comprehensive intake for patients new to Wayspring’s SUD Home solution for medical complexity or critical indicators (suicidal ideation, unsafe medication regimens, multiple medical diagnoses, complex medical diagnoses) and performs in-depth intake assessment for any member that is known to have complex needs or multiple critical indicators.
- Sends risk alerts to the treatment team and help with coordination and plan of care, as well as contingency planning for Wayspring patients.
- Provides clinical documentation of acute, chronic healthcare issues through the patient encounter in EMR system; Documentation must be accurate, high quality, and timely.
- Develops routine monitoring plan to follow any patients with critical indicators by reviewing records after scheduled interactions.
- Assists team in developing patients with complex needs care plan to improve engagement and retention in treatment.
- Conducts patient care through multiple mediums, including telehealth and in-person visits.
- Partners with team pharmacist on any medication irregularities and concerns; perform medication reconciliation if indicated during any phase of the member’s engagement.
- Reviews all patients that have a post-discharge event and are transitioned readmitted to an inpatient setting, or suffer a crisis event, and assist team in formulating a new Contingency Plan and Action Plan.
- Ensures that Wayspring’s Clinical Care Navigators understand and adhere to patients’ treatment plans and act as a clinical resource as needed.
- Provides education on patient behavioral health medical conditions, preventative care, and medication adherence compliance.
- Adhere to Wayspring information security and privacy requirements.
Requirements & Preferred Qualifications
- Master's degree in nursing
- Board certification from the AANP or ANCC as a PMHNP with a minimum of one year experience in psychiatric and primary care OR substance use treatment is preferred
- Passionate about helping the Medicaid population and individuals with substance use disorder
- Previous or current credentialing through Medicare, Medicaid, or other commercial insurance plans
- Holds and maintains an unrestricted DEA
- Willingness to be?current with all state and federal training requirements related to the prescribing of MAT
- Participates in after-hours team support rotation
- Must be able to participate in Medicaid and become credentialed with designated health plans
- Intermediate technology skills including MS Office (Logging into programs, MS Word, MS Excel, MS Outlook, MS Teams), EMR systems, etc
- Strong and reliable home internet connection
- Extensive experience in behavioral health, preferably in the field of substance use disorders, interacting directly with patients and providers.
- Experience in managed care for either behavioral health or medical case management is preferred.
- Strong organizational and communication skills are a must, a background in motivational interviewing and health coaching techniques is preferred.
- Ability to quickly identify problems and act to minimize the likelihood of adverse clinical outcomes, as well as comfortability in working with Medicaid populations.
- Comfortable working collaboratively with a multidisciplinary treatment team to achieve optimal clinical outcomes for referred patients.
