Schedule Details/Additional Information:
Monday - Friday 8am to 430 PM;1 summer and 1 winter holiday oncall per year.
This Home Health Aide position is eligible for a $2500 sign on bonus payable within 90 days from hire date. This bonus is eligible to newly hired external applicants (current AAH Team Members are not eligible at this time)
Position Requirements:
High school education or equivalent.
One year nurse's aide experience.
RN nursing students that meet state requirements.
Recognize the needs and behaviors of a variety of age groups of patients treated, understand child growth and development, i.e. understand adolescent behavior and look for signs of normal aging. Written and verbal competency of the English language.
Must be certified as a nursing assistant in the State of Illinois Must be able to travel to all of the service office boundaries in all extremes of weather on all types of road including expressways.
Must be able to assist with ambulation including turning and transferring patients up to 200 pounds.
Required to make visits to private residences which may not be accessible to people with disabilities.
If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case~by~case basis.
Purpose:
The purpose of this position is to help achieve the vision of Advocate Hospice services by performing the duties of this position in a cost effective, results oriented and customer focused manner. This position is a critical component of the Interdisciplinary team and as such meeting individual performance expectations and contribution to team performance goals is required.
