Under the leadership of the Director of Surgical Services and Surgery Coordinators, the Nurse Navigator is responsible and accountable for planning, directing, assessing and providing nursing care according to the mission, philosophy, policy and procedures of Barstow Community Hospital. The Patient Navigator assumes responsibility and accountability for the management of resources through interdisciplinary collaboration to achieve optimal patient outcomes. The Navigator coordinates research data, community physicians, hospital resources into a seamless model of access and care benefiting patients, participating physicians, and family members. The Nurse navigator is a registered nurse (RN) who helps patients navigate the healthcare system and their treatment plans. They act as a liaison between patients, their families, and caregivers, and provide a range of services, Performs other related duties
Experience: Previous 2-3 year in acute care, experience in surgery is required.
License and Certs: RN, BLS. ACLS and PALS within 6 months for new hires
Wage Scale: 54.20 - 81.07 per hour
