JOB DESCRIPTION:
SUMMARY OF POSITION
A Medication Assistant Treatment/Behavior Health Nurse provides nursing care to chronically or acutely sick patients. Monitors and adjusts medication within limits approved by the prescriber, administers medication in accordance with Federal OMT Guidelines, assesses for intoxication, and acts as a liaison between client, staff, and other care providers as needed. Provides information, education and support for clients. Promotes wellness, recovery, symptom management, and healthy lifestyle choices while recognizing and responding to relapse, managing crisis and helping clients achieve and sustain recovery.
ESSENTIAL DUTIES & RESPONSIBILITIES
- Administers medications to patients, monitors the dosage, side effects, use and documents patients’ reactions. This includes Methadone and other medication as prescribed by physician. Provides medication training to the client or client’s family or guardian to administer the client’s medications, to monitor proper levels of dosage and watch for side effects. Obtains laboratory specimens and help prescribers with patient examination and continuity of care arrangements with other providers.
- Conduct intake for recipient entering services, including explaining services, gathering information needed on intake materials, ensuring service recipient understands all forms, assuring signature on appropriate consents, and submitting materials for processing, as assigned.
- Conduct and document assessments, as assigned.
- Assist service recipients with making linkages with internal and external resources as outlined in treatment plan and in coordination with Integrated Healthcare Services. And maintain appropriate contact with referral sources including providing information on service recipient progress, behavior, and needs (with appropriate release), and maintaining ongoing consultation when appropriate.
- Assist service recipient in making discharge plans and provide follow up services as assigned.
- Attend staff meetings, clinical staffings, and other internal meetings as assigned. Participate on committees as assigned, including quality assurance functions. Maintain appropriate communication with other employees and work as an effective member of treatment team.
- Administer breathalyzers, toxicology screens and other testing as assigned.
