Position Overview:
The Care Concierge - Certified Medical Assistant serves as a dedicated liaison between health plan members residing in senior living facilities and their healthcare providers. This role ensures seamless communication, data sharing, and optimal care outcomes by coordinating care transitions, facilitating access to healthcare services, and enhancing member engagement through patient education and chronic care management. The Medical Concierge works closely with an interdisciplinary care team to support members' health and well-being, acting as a vital connector in the healthcare delivery process.
Key Responsibilities:
- Care Coordination:
- Serve as the primary point of contact for health plan members residing in senior living facilities, connecting them with healthcare providers and services.
- Ensure effective communication among providers to facilitate data sharing and achieve desired outcomes.
- Coordinate care transitions and manage referrals to ensure continuity of care.
- Collaborate with senior living facility care teams and health plans for integrated support.
- Interdisciplinary Care Team Collaboration:
- Actively participate in care planning and interdisciplinary team meetings to support the unique needs of senior living residents.
- Support the development and implementation of Individualized Care Plans (ICPs).
- Monitor and report on member progress to the care team, ensuring alignment with care goals.
- Patient Engagement and Education:
- Provide education to members about their health conditions and care plans to enhance understanding and engagement.
- Empower members to manage chronic conditions effectively through tailored guidance and resources.
- Clinical Support (Preferred for Certified Medical Assistants):
- Assist Advanced Plan Practitioners in monitoring vital signs and other clinical parameters.
- Handle prescription reorders and medication management coordination.
- Perform basic clinical tasks within the scope of certification, as needed.
Qualifications:
- Preferred: Certified Medical Assistant (CMA) or equivalent certification.
- Experience in care coordination, patient advocacy, or a similar healthcare role, preferably in a senior living or long-term care setting.
- Knowledge of chronic care management and patient education principles.
- Strong communication and interpersonal skills, with the ability to build trust and rapport with members and providers.
- Proficient in using healthcare technology platforms for documentation and data sharing.
