Central Ohio Primary Care is seeking a full-time Advanced Practice Provider (APP) for for their Spring 2025 Fellowship located in Chillicothe, Ohio.
This is an immersive 12-month training position that utilizes an inter-professional approach to prepare fellows to step into the fast-paced role of an Advanced Practice Provider (APP) with Central Ohio Primary Care (COPC). The program includes weekly didactics, precepted practice sessions, specialty rotations and other supplementary educational components that will support the APP Fellow’s transition to autonomous practice. Curriculum will have an emphasis on geriatric medicine to better attend to the adult population seen by COPC providers. The program also includes a heavy emphasis on evidence-based medicine to help Fellows develop critical reasoning skills, leadership, and clinical practice under risk-based payment arrangements. All Fellows will work under the direct guidance of physician preceptors, APP mentors, and program directors. A successful applicant for this role will be responsible for providing services independently and collaboratively, including initial and ongoing patient assessments, diagnoses, treatment plans, etc.

Clinical Job Functions:
- Develop patient care treatment plans by obtaining medical histories and examining patients during appointment -- this includes providing a diagnosis and establishing patient care needs to ensure desired outcomes of care.
- Perform routine or annual physical examinations including treating or referring patients for primary care conditions, as appropriate.
- Educate patients and caregivers on conditions, outcomes, or treatment plans.
- Consult with Physicians and clinical staff regarding issues, treatment plan and progress.
- Prescribe medication, dosages, routes, and frequencies based on individual patient needs.
- Order, perform and/or interpret results of diagnostic tests when necessary.
- Participate in monitoring of patient outcomes and assure quality of care by adhering to professional standards and procedures set by federal, state and company requirements.
- Maintain accurate records of assessments, follow-ups, and evaluations.

Education, Licensures & Certifications
Required: Master's Level Degree or equivalent combination of education and experience.
Required: Must be considered a Certified Nurse Practitioner by a nationally recognized board in the state of Ohio OR a Physician Assistant by nationally recognized board in the state of Ohio before the Fellowship begins.
Required: Must possess an active license for the state of Ohio before the Fellowship begins.
