Your Impact
- Be a key player on a collaborative team, working with Primary Care Providers, Medical Assistants, and Behavioral Health professionals.
- Offer guidance to patients by responding to health inquiries over the phone, drawing on your knowledge of illness and injury to carefully assess and advise without in-person contact.
- Engage in inbox management using our Electronic Medical Record (EMR) system, ensuring efficient communication and follow-up with patients.
- Provide vital case management and care coordination, including triaging patients, overseeing immunization administration, and ensuring smooth transitions from hospital care to nurse visits.
- Educate patients on health management, medication use, and wellness strategies, fostering understanding and promoting proactive care.
- Monitor controlled medication refills and collaborate closely with providers to review test results and ensure timely patient follow-up.
- Use nationally recognized triage protocols to assess patient needs, developing care plans based on phone conversations and medical record reviews.

Who You Are
- Education & Licensure:Registered Nurse (RN) with an active license.
- Experience: 3-5 years of experience in a primary care or specialist care setting.
- Proactive and confident communicator (written and verbal): Foreshadows and elevates concerns as they arise, unafraid of professional directness both with colleagues and patients.
- Detail-oriented with excellent organizational skills: Ability to juggle and prioritize multiple high-priority tasks and handle a fast-paced work environment.
- Compassionate, authentic, and patient focused: Supports patients through kindness; demonstrating understanding for others and contributing to the creating a community of belonging.
- Technically savvy:Proficient with Microsoft Office Suite (particularly PowerPoint and Word) or related software and comfort with EMR systems.
- Effective communicator and cross collaborator:Ability to establish and maintain positive relationships with patients, team members, and stakeholders across the organization.
- Excellent team player: You enjoy collaborating with others and being a part of a strong team dynamic.
- Driven and self-motivated: Capable of working independently, possess a level of initiative and enthusiasm to help drive results. Actively identifying better ways of working. Uses resourceful to achieve the best outcomes for patients, the team, and the organization.
- Strong analytical and problem-solving skills:Able to comprehend, interpret, and apply the appropriate sections of applicable laws, guidelines, regulations, ordinances, and policies.
- Trustworthy:Ability to maintain confidentiality and handle all patient information in accordance with HIPAA regulations.
- Able to work for prolonged periods sitting at a desk, working with computers, and handling administrative tasks.
