Bargaining Unit: EAP

Rate of Pay: $68.46/hour + DOE

Summary
The nurse navigator manages the complex care processes from the time of diagnosis, through the treatment phase, and post recovery phase for each individual patient. This individual is often the first contact for the diagnosed patient and is seen as the “dot connector” for ongoing care. The objective is to reduce gaps in the care process, streamlining care, while the patient moves from one discipline to the next.

Essential Duties and Responsibilities

Builds awareness of Oncology Nurse Navigator within TFHD care providers.
Responds to criteria based referrals ASAP and not longer than 24 hours.
Gathers pertinent information about the patient’s medical, psychosocial, functional and financial status.
Communicates with multidisciplinary team (physicians, nurse practitioners, physician assistants, nurses, therapists, social workers, etc) as needed to complete assessment.
Demonstrates an ability to assess patient needs, make appropriate referrals face to face or via telephone as indicated for ancillary support services and programs.
Acts as the single point of contact for new cancer patients and families.
Helps schedule tests, procedures, appointments, and treatments.
Helps develop a treatment plan that is understandable and feasible to the patient.
Helps patients and caregivers navigate the healthcare system.
Work across healthcare disciplines to ensure timely appointments, results reporting, and communication on patient care and follow up.
Educate/reinforce education with patients and families on disease, treatments, side effects, adverse reactions, and reportable signs/symptoms based on multidisciplinary care plan.
Directs patients and families to available community resources and supportive services.
Assists patients to overcome barriers to care, e.g., transportation, childcare, financial counseling.
Coordinates discharge planning with the patient’s family, social worker, oncologist, home health care, hospice care, palliative care, and primary care physicians.
Links patients and all other care providers—both inside and outside your healthcare system.
Refers patients to supportive care services, including dietitians, socials workers, and financial counselors as appropriate.
Helps eligible patients access appropriate clinical trials.
Assists in the development a Survivorship Care plan.
Demonstrates a high level of understanding of oncology illnesses and treatment modalities.
Organizes and facilitates multidisciplinary care conferences.
Ability to set priorities and meet deadlines.
Ability to work independently without close supervision.
Demonstrates System Values in performance and behavior.
Complies with System policies and procedures.
Other duties as may be assigned.

No supervisory responsibilities.

Minimum Education/Experience
Associate's Degree Certificate of completion from college or vocational school from a National League of Nursing (NLN), Commission on Collegiate Nursing Education (CCNE) or equivalent accredited school of nursing. The nurse shall have completed a nursing education program that included clinical experiences with actual patients under the supervision of qualified faculty.

2-3 years relevant experience

Required Licenses/Certifications
California Registered Nurse (RN) - Upon hire

Oncology Certified Nurse (OCN) - Within 1 year of hire into job

Basic Life Support (BLS) for Healthcare Providers - Upon hire

BLS: Employee will be enrolled in the Resuscitation Quality Improvement (RQI) Basic Life Support (BLS) Entry or Prep Curriculum (depending on their previous BLS certification). - Within 3 months of hire into job

Other Experience/Qualifications

Required:

A minimum of one year experience as an oncology nurse

Preferred:

Three years of oncology nurse experience
Oncology nurse navigator experience
Oncology Certified Nurse (OCN)
Bachelor's degree (BA, BS, BSN) from four-year college or university
