Job Summary:
Are you passionate about helping individuals improve their health and well-being? Join our team as a Nurse Care Manager, where you’ll work under the supervision of the Health Homes Project Director to coordinate healthcare services for our participants.
You’ll oversee Wellness Coaches and ensure that all individuals receive comprehensive care through collaboration with multidisciplinary teams. This role provides a unique opportunity to apply your nursing skills while making a positive impact on the lives of our clients. In this role, you’ll coordinate wellness plans, organize healthcare services, and offer direct support to program participants, helping them achieve personal health goals and independent living.

Key Responsibilities:
- Coordinate healthcare services: Serve as a liaison for participants, organizing initial appointments, conducting health screenings, and processing referrals for additional services.
- Collaborate on wellness plans: Work with a multidisciplinary team to develop and implement wellness plans based on participants’ strengths, needs, and preferences.
- Supervise and support Wellness Coaches: Provide guidance to Wellness Coaches, review interventions, and assist with coaching quality improvement efforts.
- Monitor participant progress: Regularly assess participants’ health, evaluate their progress on wellness goals, and update the team with key information.
- Record-keeping and documentation: Maintain accurate participant health records in the electronic medical record system (EHR), and ensure compliance with organizational protocols.
- Engage participants in healthy living: Educate and support participants in developing healthy living skills and achieving wellness plan goals.
- Collaborate with external providers: Coordinate care with external community providers to ensure comprehensive support for participants.
Education and/or Experience Qualifications:
- Licensure as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) is required.
- Certification in wellness-based programs is preferred.
- Established experience in wellness coaching/domains is required.
Required License/Certification:
- Licensed Practical Nurse (LPN) or Registered Nurse (RN).
Additional Qualifications:
- Strong knowledge of nursing principles and healthcare systems, particularly within behavioral health and general health care.
- Proficiency in using electronic health record (EHR) systems and ability to maintain detailed and confidential records.
- Excellent interpersonal skills and the ability to communicate effectively with participants and multidisciplinary team members.
- Ability to manage multiple tasks and adapt to changing priorities.
- Strong organizational and time management skills.
Supervisory Requirements:
- Provide guidance and limited supervision to Wellness Coaches in collaboration with the Health Homes Project Director.
- Communicate the strategic direction of the organization and encourage participation from team members.
- Reinforce a team approach and support input from team members at all levels.
- Handle difficult team member situations with discretion and respect, seeking Human Resources advice when needed.
- Champion changes and effectively manage the implementation of new ideas and initiatives within the department.
