This position is located on our Onamia MN campus

12 hours rotating shifts with every other weekend and rotating holiday expectations

Summary
Responsible for providing basic medical attention to patients and assessing their wellbeing as part of a larger medical team. Duties include, but not limited to, checking vital signs, recording a patient's medical history, assisting patients with hygiene, medication administration including- PO, IM, SQ medications.

Job Duties

Effectively Utilize Nursing Process:

Monitor patient's vitals and record them in their file*
Maintain the cleanliness and comfort of each patient, including bathing, feeding and dressing*
Assist nursing and medical staff with basic medical tasks, such as administering medication*
Update patient records in a confidential manner
Evaluate the patient's wellbeing, reporting any needs to Charge Nurse *
Explain treatments and procedures to patients and families*
Change dressings and bandages, ensuring patients are healing properly*

Safety Awareness:

Reports safety hazards.*
Cleans equipment, spills, etc., as necessary.*
Keeps work area clean and neat.
Reports accidents or incidents promptly.*

Quality Work:

Sets and meets individual/professional goals.
Completes all documentation in full at all times.*
Performs with minimal supervision.*
Uses work hours efficiently, minimizes overtime.*
Demonstrates willingness to work extra shifts when needed.
Assists team members when requested and time allows.*

Initiative:

Completes duties assigned without hesitation in a timely manner.*
Accepts direction well.*
Works independently as position requires.*
Demonstrates a willingness to train new staff.*

Customer Service:

Is courteous and friendly, uses a quiet, respectful tone of voice when speaking with patients, visitors, physicians and others.*

Nursing Process/Patient Care:

• Conducting a focused assessment of the health status of an individual patient through the collection and comparison of data to normal findings and the individual patient's current health status, and reporting changes and responses to interventions in an ongoing manner to a registered nurse or the appropriate licensed healthcare provider for delegated or assigned tasks or activities;*
• Participating with other health care providers in the development and modification of a plan of care; determining and implementing appropriate interventions within a nursing plan of care or when delegated or assigned by a registered nurse; *
• Implementing interventions that are delegated, ordered, or prescribed by a licensed health care provider;
• Assigning and monitoring nursing tasks or activities to unlicensed assistive personnel;
• Providing safe and effective nursing care delivery; *
• Promoting a safe and therapeutic environment; *
• Advocating for the best interests of individual patients; *
• Assisting in the evaluation of responses to interventions; *
• Collaborating and communicating with other health care providers; *
• Performs treatment interventions requiring a physician's order and LPN licensure for administration, i.e. medication administration, medical treatments.*
• Monitors and enforces utilization of proper nursing techniques and procedures.*
• Attends monthly department meetings, 50% are required. All meetings not attended; minutes are consistently read.

Communications/Documentation:

Identifies needs, instructs others and actively supports the role of patient advocate/liaison by communication and documentation of patient information to the proper resources/agencies.*
Listens carefully without interrupting, gives others full attention.
Communicates with co-workers in a professional manner at all times.*

Required Education and Experience

Graduate of accredited school of nursing with current licensure as a Licensed Practical Nurse in the state of Minnesota.
Knowledge of nursing principles, policies, procedures and medical ethics equivalent to years of experience.
BLS or obtain within first month of employment

Preferred Education and Experience

Prefer minimum of one year's experience in nursing in mental health.

Position Type/Expected Hours of Work
This is a position in a health care facility open 24 hours a day, 365 days a year. Rotating shifts, as well as holiday and work during inclement weather, will be required.
