As the MDS Coordinator (RN) or (LVN)your focus will be Medicaid and Medicare reimbursement. You will be responsible for the coordination and completion of clinical assessments, supporting documentation, care planning, and transmitting MDS 3.0, all as required by federal and state regulations.You will participate in and direct the delivery of patient care in accordance with facility clinical systems, regulatory requirements, and physician orders. MDS Coordinator responsibilities:
- Maintaining MDS/Care plan schedule for interdisciplinary team
- Coordinating preparation of MDS and Care Plan as per regulatory requirements
- Tracking Medicare resident benefit days, validating daily Medicare census, and coordinating this information with management to assure accurate billing
- Completing responses to requests for additional information from the intermediary and appeals decisions, when necessary, within established time frames
- Ensuring preparation of Medicare denial letters for each resident discharged from Medicare coverage within time limits imposed by Medicare guidelines
- Participating in the ongoing evaluation of documentation required for Medicaid reimbursement
- Investigates, monitors, and communicates known or potential sources of suspected infections to determine sources and minimize spread.
- Demonstrates strong knowledge of standard methods and criteria for identifying nosocomial infections using an epidemiological approach that consists of surveillance, data collection, and trending for identification.
- Instructs and counsels patients and staff on Infection Control issues and concerns.
- Collaborates with other members of the team to follow up on exposure incidents.
- Serves on the Quality Committee.
- Provides Infection Control monthly summaries and any additional Infection Control Alerts to the facility leadership and Quality Committee monthly and to include an annual summary.
- Utilizes computerized documentation systems to access relevant patient data.
- Provide new staff orientation and ongoing mentoring regarding infection control practices.
- Follows Standard Precautions and appropriate CDC recommended isolation categories using personal protective equipment as required.
- Ensures process in place to identify and monitor those patients requiring isolation for an infectious disease.
- Responsible for the implementation of the Infection Control Policy manual.
- Provide ongoing education to nursing and ancillary staff on Infection Control practice, current CDC guidelines and regulatory standards.
- Coordinates annual review, revision, and evaluation for the Infection Control plan
- Participates in staff meetings, patient care conferences and other committees as required
- Participates in mandatory department and facility in-services and education programs
- Maintains on-going education to improve job performance
- Demonstrates sensitivities to culturally diverse patient population
- Proactively seeks team building and positive working relationships with all facility and medical staff
- Participates in facility or departmental performance improvement activities, and seeks opportunities to improve department and inter-department processes
- Demonstrates flexibility regarding assignments
- Perform special assignments as needed/requested
MDS Coordinator requirements:
- Current RN or LVN license in the state of Texas
- 2+ years of experience in skilled nursing facility with previous experience in completing MDS’s and Care Plans as an MDS Coordinator
- Demonstrated proficiency in the RAI / PPS processes
- Strong attention to detail, documentation, and resident assessment skills
- Good communication, clerical, organizational, and computer skills
