SUMMARY
Provide skilled nursing services to patients in their homes as ordered by the primary physician. Function under the supervision of the Registered Nurse Case Manager. Not responsible for completing admissions, pronouncing deaths, or preparing re-certification documents, which are the sole responsibility of the Registered Nurse Case Manager.
ESSENTIAL DUTIES AND RESPONSIBILITIES

• Provide nursing care to clients through implementation of the nursing care plan prepared by the Registered Nurse Case Manager (RN Case Manager)
• Contribute to the formulation/revision of the nursing care plan under the direction and supervision of the RN Case Manager or his/her designee
• Implement orders for medications and treatments as prescribed by the patient’s primary physician and all aspects of the nursing care plan
• Instruct clients/caregivers in care techniques or other aspects of care as ordered by the physician or as outlined in the nursing care plan
• Work with the family members and other members of the health care team to meet emotional needs of the client and family members
• Maintain a safe environment for the client
• Observe the patient for changes in condition and report these changes to the RN Case Manager
• Notify the RN Case Manager when there is a need for an assessment
• Accurately complete required documentation within 24 hours of patient visit
• Complete, maintain, and submit accurate and relevant clinical notes regarding client condition and all care and treatments given
• Seek and obtain sufficient information to function effectively in the health care setting assigned
• Assist field staff with clinical issues during times noted above
• Complete all mandatory education requirements
• Attend and participate in all required agency meetings

QUALIFICATION REQUIREMENTS

• Graduate of an approved school of Practical Nursing
• Current licensure as a Licensed Practical Nurse by the state in which you are employed
• A minimum of two (2) years of experience working as a Licensed Practical Nurse including some medical/surgical experience and/or an acute care or home care setting preferred
• CPR certification
• Proficiency in agency IT programs or able to demonstrate proficiency in all tools for documentation
• Able to meet the physical demands of providing nursing care, ambulating patients, transferring patients, assisting with exercises, etc.
• Able to transfer a patient as needed according to proper techniques of body mechanics to promote safety of patient and staff
• Able to follow up as needed and appropriately with multidisciplinary team and management staff pertaining to any concerns regarding patient care
CERTIFICATES, LICENSES, REGISTRATIONS
Must have and maintain in good standing professional license, certificate, or registration, as applicable.

WORK ENVIRONMENT

Employee will be required to drive daily for meetings and/or visits to residential homes. Employee will work in varied residential environments based on assigned case load. While performing the duties of this position, the employee may travel by automobile and be exposed to changing weather conditions.
