Under the direction of the Manager, this position is responsible for the planning and the management of the nursing care of the assigned caseload of patients in home or long term care in collaboration with the interdisciplinary team.
The RN Case Manager will:
- Demonstrate critical thinking, clinical judgment and analysis in the care of patients and families.
- Incorporate ethical principles and hospice standards in the care of patients and families experiencing life-limiting, progressive illnesses as well as identifying and advocating for the wishes and preferences of patients and families.
- Demonstrate knowledge, attitudes, behaviors, and skills that are consistent with the performance standards, code of ethics and scope of practice for hospice nurses.
- Facilitate dialogue with patients and families experiencing life-limiting progressive illness and bereavement to actively address patient and family goals in collaboration with the interdisciplinary team and community.
- Utilize resources necessary to enhance quality of life for patients and families experiencing life-limiting, progressive, illness and bereavement through knowledge and collaboration within the health and human service systems.
- Demonstrate cultural competence by respecting and honoring unique values, diversity, and characteristics of patients, families, and colleagues in hospice and palliative care.
- Coordinate training for patient, family, other team members and community through formal and informal education related to living with and dying from a life-limiting, progressive illness and the grief process1.
- Demonstrate the use of effective verbal, non-verbal, and written communication with patients, families, other team members, and community in order to meet the patient’s and families’ hospice goals and needs1.
Location:
All employees assigned this job title must flex their work location within and between all Community Hospice and Palliative Care locations as needed.
Primary Responsibilities:
- Collaborates with the interdisciplinary team to identify and meet patient/family identified needs.

- Participates in the interdisciplinary team meeting and considers the perspectives of all disciplines in the care planning process.

Participates in organizational operations:
- Operational systems are maintained in regard to Federal and State Hospice Standards.
- Compliance with Community Hospice Pharmacy and State Regulations in regard to obtaining, disposing, dispensing pharmaceuticals.
- Takes initiative in addressing areas of need noted regarding facility operations/services and advising the appropriate department for resolution.
- Demonstrates professional relationships with internal and external customers.
- Promotes organizational mission, values and image within Community Hospice and Palliative Care and the community.
- Demonstrates appropriate dress and grooming.

- Participates in organization Quality Assurance Performance Improvement (QAPI) as appropriate.

- Patient occurrence reports are completed accurately and submitted timely to the manager.

- Developing basic knowledge of Hospice Medicare/Hospice Medicaid and other reimbursement sources.
- Developing areas of expertise in hospice nursing skills.
- Pursuing professional growth through academic and continuing education programs.
- Assisting in the orientation and mentorship of new employees.
Additional Duties:
- May facilitate the IDG meeting in the absence of the manager
- Performs other duties as assigned.
Required Licenses/Certifications:
- Current licensure as Registered Nurse in the State of Florida.
- Current CPR certification.
- Current State of Florida Driver’s license with driving record acceptable to Community Hospice and Palliative Care insurance carrier.
Qualifications:
Education/Experience:
Any combination of education and experience of required skills and knowledge for successful performance is qualifying. Typical qualifications are equivalent to:
- Possession of a two year Associate Degree in nursing or a Bachelor’s Degree in nursing from an accredited school of nursing program,
- One or more years Registered Nurse experience with at least one year hospital medical-surgical/ oncology experience preferred or
- Five years Licensed Practical Nurse experience within the past five years and at least one year hospital medical–surgical/oncology experience preferred.
Knowledge of:
- Appropriate educational principles and methods as applied to hospice nursing.
Skilled at:
- Effective communication and teamwork.
- Demonstration of confidence and ability to manage inpatient facility functions and patient care needs on his/her shift
- Working long and occasionally irregular hours as needed.
Professional Development:
Community Hospice and Palliative Care adheres to local, state and federal mandated educational and continuing education requirements.
- Maintains competency by participating in in-service and continuing educational programs. The organization’s Professional Development Department will inform and provide you with a monthly Education Calendar indicating dates and times for all programs.
- Attends and participates in internal staff development programs. Obtains continuing education as required by regulations.
- Assists in training and education of other healthcare staff members.
- Complies with accepted ethical conduct and professional standards of nursing practice as set forth by the American Nursing Association and follows the guidelines as set forth in the Nurse Practice Act.
