Job Description
LOCATION: Lancaster, PA
HOURS: Full-Time, Day Shift (32 hours/week). M0F daytime hours, with occasional evenings and/or weekends for outreach activities.
POSITION SUMMARY:
The Nurse Navigator is accountable for providing clinical navigation with the LGH healthcare system for Women Internal Medicine complicated patients with multiple chronic diseases and are high-risk. The Nurse Navigator performs clinical and administrative functions to support and execute care management for this patient population and those with new disease diagnosis.
ESSENTIAL FUNCTIONS: Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:
- Knowledge of pathophysiology of specific disease processes and understands chronic disease treatment modalities and symptom management.
- Identify and mitigate barriers to care such as financial, communication information, system navigation, and psychological
- Remain a point of contact for the patient along the continuum of care.
- Individual assessment of patient's ability to navigate through the healthcare system and identify potential barriers. Guide patient through the healthcare system including appropriate referrals to specialties, social work, agencies and financial counseling.
- Ability to communicate among specialty practices with the healthcare organization – build relationships with physicians and staff
- Identify and implement methodologies for continuous quality improvement specific to chronic disease and women’s health related issues
SECONDARY FUNCTIONS: The following duties are considered secondary to the primary duties listed above:
- Take leadership role in building a primary care home including formal applications such as PCMH
- Support integrative programs provided by the Women Specialty Center; ex. Diabetes Prevention Program and MBSR
- Foster proactive, preventative approach to patient care rather than reactive
- Other duties as assigned.
JOB REQUIREMENTS
MINIMUM REQUIRED QUALIFICATIONS:
- Current licensure as a Registered Professional Nurse, issued by the PA State Board of Nursing required
- Bachelors of Science in Nursing (BSN)
- Experience in chronic care management
PREFERRED QUALIFICATIONS:
- Certification in Chronic Care Populations
- Advanced Care Planning Certification
