Hospice Home Health Aide - Vigil

Hospice helps patients and their families prepare physically, emotionally and spiritually for the end of life. Our focus is on providing comfort, not cure; on family, not just the patient; and on quality of life, not duration. Cleveland Clinic’s hospice team, including physicians certified in hospice and palliative medicine, nurses, social workers, chaplains, home health aides and volunteers, care for patients where they call home (home, nursing facilities, assisted living facilities, etc.).
A caregiver in this position works Monday-Friday, 4 hour shifts 5 days a week. The hours could vary 10:00PM -2:00AM or 9:00PM- 1:00AM.
A caregiver who excels in this role will:
- Perform basic patient care activities.
- Assistwith procedures ordered by physician and supervised by an RN, PT, OT or ST as appropriate.
- Assist patients with tending to personal care, activities of daily living and transfers/transport.
- Report and document abnormal assessment findings or changes in physical, mental and emotional conditions to nursing staff.
- Assist with keeping the patient's environment clean and orderly.
- Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.
- Escalate change in patient condition to patient’s physician as appropriate.
Minimum qualifications for the ideal future caregiver include:
- High school diploma or GED.
- Certification and registered with the State of Ohio Nurse’s Aide Registry (STNA). Please include your STNA number on your application.
- One year of clinical experience as an aide in an acute, rehab or skilled nursing setting.
- Valid Ohio driver’s license and proof of automobile insurance coverage with $100,000/$300,000 coverage.
Preferred qualifications for the ideal future caregiver include:
- Basic Life Support (BLS) certification through the American Heart Association (AHA). This will be required within the orientation period prior to providing independent patient care.
- Hospice and Palliative Care Nurse Assistant (CHPNA) certification.

Physical Requirements:
- Must be ambulatory, able to climb stairs, pull, push, and facilitate movement of the patient.
- For field staff only: Able to drive to patients’ homes and/or facilities.
- Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
- Physical Demand requirements are in excess of those for Light Work.
Personal Protective Equipment:
- Follows standard precautions using personal protective equipment as required.
