CHI St. Alexius is looking for a Full Time RN Health Coach to join the team!
The RN Health Coach is responsible for utilizing the professional nursing process to provide individualized nursing care across the life span in an ambulatory setting. They care for patients in all phases of preventative care, health maintenance, diagnosis, treatment, and follow-up. The RN Health Coach in the primary care clinic assists with the coordination of patient care across the continuum by collaborating with physicians, APP’s, staff and other professionals. The RN HC is a liaison with specialty practices, in-patient facilities, and serves as a link to community resources. The Coach also ensures the patients care plans are updated. The RN HC works with the clinical team and manages the disease registries by identifying and following up with patients not meeting clinical goals, based on evidenced-based, Clinical Practice Guidelines. The RN HC also works with the clinical team and may assist or conduct the pre-visit chart review to prepare for a comprehensive patient visit by identifying preventive health care needs based on the approved guidelines.
The RN HC works with patients and families, using education, goal setting, action planning, problem solving and motivational interviewing to support health behavioral change. They support quality improvement and transformation of the health care delivery system.
The RN HC demonstrates excellent communication skills with patients, family members and others on the healthcare team and engages the patient and family into the care team. Demonstrates independence, assertiveness and critical thinking when working with patients, co-workers; and contributes leadership to the healthcare team. Demonstrates the ability to take charge, take unpopular stands if necessary, face difficult situations, and is looked to for direction from others. Documents clearly, able to problem solve and is self-motivated. Works autonomously and independently with little direction. Able to incorporate teaching/learning principles and adapt teaching to age group. Able to facilitate groups and implement projects. Able to establish priorities. Has strong flexibility and organizational skills. Knowledgeable about reimbursement for services provided. Demonstrates and applies continuous quality improvement knowledge and skills.

QualificationsEducation: Bachelor’s Degree in nursing preferred.
Licensure: Currently licensed with the applicable state nursing board and/or possess
multistate licensure privileges. Functions within the legal scope of practice. Must hold a valid
drivers license.
Experience: Minimum of three years of professional nursing experience in a clinical care setting preferred
