Location: Butte/Dillon Montana (Hybrid In-home/remote)
Position Summary: The Clinical Manager establishes and implements Granite Peak Home Health Skilled Home Care Processes. The Clinical Manager or qualified designee provides oversight of all patient care services and personnel, including making patient and personnel assignments, coordinating patient care, coordinating referrals, ensuring that patient needs are continually assessed, and ensuring the development, implementation, and updates of the individualized plan of care.
Qualifications:
- Minimum of 2 years of nursing experience in Home Health (preferred).
- Associate’s degree in nursing (required); bachelor’s in nursing (preferred).
- Acceptance of the agency’s philosophy and goals.
- Ability to exercise initiative and independent judgment.
- Management experience and knowledge of Montana Home Care License, CMS Skilled Home Care License, and federal, state, and insurance reimbursement programs.
- Basic computer skills and proficiency with web-based software and electronic health records (Wellsky).
- Must pass a criminal background check by the Department of Human Services and not be on the OIG exclusions list.
- Must have a cellular phone for work-related use.
Principal Responsibilities:
- Reporting: Reports to the Home Health Director.
- Oversight: Provides oversight of all patient care services and personnel, directing the clinical activities of the branch.
- Clinical Supervision: Provides direct oversight of assessment, planning, implementation, and evaluation of patient and family/caregiver care by all clinical personnel.
- Staff Management: Leads, manages, and holds Granite Peak Home Health clinical staff accountable for compliance with regulations, policies, and procedures, ensuring excellent, quality, and timely care.
- Meetings: Schedules and conducts weekly IDT meetings with assigned staff, following EOS processes.
- Policy Development: Develops and implements processes, policies, and procedures, coordinating and overseeing all patient services provided by clinical personnel.
- Budget Assistance: Assists the Community Services Director in preparing and administering the Granite Peak Home Health budget.
- Operational Monitoring: Interprets operational indicators to monitor census changes and adjusts staffing levels, revenues, or expenses accordingly.
- Compliance: Ensures compliance with the Granite Peak Home Health Employee Handbook and policies.
- HR Functions: Hires, evaluates, and terminates organization personnel.
- Orientation and Training: Develops, implements, and evaluates orientation for new Granite Peak Home Health personnel and plans in-service and continuing education programs.
- Performance Evaluation: Conducts quarterly conversations and annual performance evaluations of supervised staff.
- Record Maintenance: Collaborates with the HIM Coordinator to maintain accurate patient records and monitors statistics and reports for evaluation and reporting.
- Supply Management: Ensures adequate and appropriate supplies and equipment for patient services.
- Quality Assurance: Assists with evaluating performance via the QAPI program, ensuring staff productivity and safe, quality home health services.
- Goal Development: Participates in developing LB Homes goals and administers Granite Peak Home Health policies and procedures.
- Regulatory Compliance: Collaborates with LB Homes HR department to ensure compliance with local, state, and federal laws regarding licensure and certification.
- Industry Awareness: Stays informed about nursing and home health care changes, adjusting procedures accordingly.
- Regulatory Knowledge: Remains updated and familiar with the rules and regulations of third-party agencies, state licensure, and Medicare standards.
- Referral Promotion: Promotes referrals to Granite Peak Home Health.
- Staff Development:
- Identifies training needs and opportunities for professional development among staff.
- Creates and implements development plans to enhance staff skills and competencies.
- Mentors and supports staff in achieving their professional goals.
- Organizes workshops, seminars, and other educational activities to foster continuous learning.
- Encourages a culture of continuous improvement and professional growth within the team.
- Performance Metrics:
- Establishes and monitors key performance indicators (KPIs) for clinical staff to ensure high-quality patient care.
- Tracks and analyzes patient outcomes, satisfaction scores, and other relevant metrics to identify areas for improvement.
- Reports on performance metrics to senior management and uses data to drive decision-making and process improvements.
- Ensures compliance with all regulatory and accreditation standards through regular audits and reviews.
