Job Description
The role of Licensed Practical Nurse is an important position on our team which works with other staff in achieving high quality, patient centered care. This position requires the desire and ability to work with all types of patients from pediatrics to geriatrics. The successful LPN also possesses the ability to work with a multidisciplinary team of Medical Assistants, LPNs, Paramedics, and Radiologic Technologists. The LPN often collaborates with the Registration team, Provider team, and other departments to provide seamless, high quality care to our patients. This position requires effective communication skills and the ability to work calmly under stressful conditions within time constraints. This LPN must be willing to collaborate with team members from other locations to ensure consistency in the care given across locations. The LPN must be willing to travel to other Urgent Care sites in order to acquire training and provide coverage as needed. The LPN will be required to achieve patient satisfaction scores measured by surveys and reported monthly.
Other duties include, but are not limited to:
Clinical Patient Care:
• Delivers and maintains optimum patient care through implementation of medical and nursing orders according to established UCC policies and procedures.
• Assists the provider in evaluating patient responses to clinical condition and effectiveness of treatment. Also assists provider in assessment of the physical, psychological and social assessment of assigned patients.
• Effectively communicates, obtains and interprets information in a manner appropriate to the age and developmental level of the patient.
• Administers prescribed therapy and medications safely while using 2 patient identifiers and as ordered and documents in EMR appropriately.
• Ensures all charges are entered into the patient’s record prior to discharge.
• Review discharge information with patient and assess for understanding as needed.
• Adheres to National Patient Safety Goals as they apply to Ambulatory Care setting.
• Provides competent assessment, care and treatment appropriate to the age of patients served as detailed and determined through the unit specific Scope of Care, Competency/Skills Checklists, and In-service Training.
• Follows all safety precautions on the clinical floor and in the laboratory. Reports unsafe conditions, patient risk, or missed service opportunities in VH Risk Qual.
• Participates effectively in evaluating equipment; maintains stocking of supplies.
• Demonstrates effort to improve patient satisfaction scores to clinic standards as noted by specific acts, conversations, and/or feedback.
• Based on previous assessments, recognizes changes in the patient’s condition and communicates these changes to the team leader, resource nurse and/or physician.
• Demonstrates Personal Responsibility To The Daily Operation Of The Urgent Care Center/Occ Health:
• Assists with the orientation of new staff members to the unit making them feel welcomed; will be measured by peer review.
• Completed cross-training and signed-off as competent in all skills related to Urgent Care including (but not limited to) triaging, EKG, phlebotomy, laboratory skills, Code STEMI protocol, ear lavage, casting/splinting, medication administration, and appropriate documentation.
• Adheres to VH policies including, but not limited to: VH Attendance policy for arrival and departure times at work, VH infection control policy regarding food in the work place, and VH internet/cell phone policy (HR507).
• Expresses appreciation, compliments and encourages others.
• Completes all daily tasks assigned, including opening and closing duties in clinical and lab areas. Completed cross-training and signed-off as competent in the following Occupational Health areas: Urine Drug Screen, Hair Collection, Oral Swab Collection, BAT, Audiometry, Spirometry, Titmus, and working knowledge of Occupational Health regulations.
• Functions as a qualified team leader by assisting where needed and bringing a high-level of professionalism to work environment. Demonstrates adaptability to changing situations and clinic needs.
Growth and Development:
• Identifies own educational needs; sets goals for self-growth, and plan for annual accomplishment of goals. Maintains current certifications such as BLS, licensure, and others. Also demonstrates ownership of self-development by seeking out training.
• Assumes additional responsibilities as requested. Regularly contributes ideas to improve clinic efficiencies in workflow.
• Participates in VH initiatives; such as Language of Caring, LEAN, etc.
• Is adaptable to changing situations and unit needs.
Patient Satisfaction:
• Communicates wait times with patients every 20 minutes; provide items to ensure comfort during periods of extended waits.
• Works with Clinical Coordinator and Director to initiate and maintain initiatives that will result in patient satisfaction scores in the 75th percentile or greater by Press Ganey.
Laboratory Testing personnel must:
• Follow the laboratory’s procedures for specimen handling and processing, test analyses, reporting, and maintaining records of patient results
• Maintain records which demonstrate that proficiency testing samples are tested in the same manner as patient specimens
• Adhere to the laboratory’s quality control policies and documenting all QC activities, instrument and procedural calibrations, and instrument maintenance
• Follow the laboratory’s policies whenever test systems are not within the laboratory’s established acceptable levels of performance
• Be able to identify problems that may adversely affect test performance or reporting of test results and either correct the problem or notify the appropriate supervisor.
• Document all corrective actions taken when test systems deviate from the laboratory’s established performance specifications.
• Maintain state licensure for laboratory personnel if working at an Urgent Care in West Virginia.
Education
High School Diploma or GED required for lab duties
Certification & Licensures
LPN Nursing License required *
BLS Certification (Basic Life Support) - American Heart ‘Healthcare Provider’ (HCP) - AHA approved required **
Additional Certification/Licensure Requirements
* Licensed in the State of Virginia or West Virginia, based on specific job posting, prior to commencing employment. Licensure to practice in the secondary state of assignment is required within 90 days of employment.
** New hires must have American Heart Association (AHA) appropriate certification prior to completion of orientation.
Qualifications
•Must be willing to work in all VH UC/OH/QC clinics; carry licenses that allow for the capabilities; and be flexible with last minute assignments.
•Knowledgeable in the cognitive, physical, emotional and chronological stages of human growth and development.
•The individual must demonstrate knowledge of the principles of growth and development over the life span, possess the ability to assess data reflective of the patient’s status, and interpret the appropriate information needed in order to provide care for the age group of patients served.
•Be cross-trained in all areas as designated by Urgent Care Center/Occ Health Manager.
