Graduate from an approved school of nursing. Current & valid licensure to practice as a Registered Nurse by the Arkansas State Board of Nursing as demonstrated by renewal & expiration dates. Must complete CPR certification upon employment.

One to three years of clinical experience in a clinical practice area to which assigned. Current working knowledge of discharge planning, utilization review, utilization management, case management, performance improvement, managed care reimbursement or nursing management preferred. An understanding of pre-acute and post-acute venues of care and post-acute community resources. Strong organizational and time management skills, flexibility, as evidenced by capacity to prioritize multiple tasks and role components. Ability to work independently and exercise sound judgment in interactions with physicians, payers, patients and their families. Experience in home health, discharge planning, utilization review or nursing management preferred.

A collaborative position that supports the patients, nurses, social workers, physicians, other practitioners, caregivers and the community. The RN CM communicates and facilitates care along a continuum through effective resource coordination to achieve optimal health, access to care, and appropriate utilization of resources, balanced with the patient’s right to self-determination. The role is to serve as the driver of patient centered care. Ensures the multidisciplinary team, including physicians, work together to eliminate barriers to care and timely discharge. The role extends beyond the hospital walls into the community to achieve better transitions of care. The position serves to enhance the quality of clinical outcome and patient satisfaction while managing the cost of care and providing timely and accurate information to payers. The role integrates and coordinates utilization management, care facilitation and discharge planning function. Accountable for a designated patient caseload and plan effectively in order to meet patient needs, manage the length of stay and promote efficient utilization of resources. Specific functions within this role include: Facilitation of the collaborative management of patient care across the continuum, intervening as necessary to remove barriers to timely and efficient care delivery and reimbursement; application of process improvement methodologies in evaluating outcomes of care; support and coaching of clinical documentation efforts and serving as a clinical resource for CDI ensuring that documentation accurately reflects severity of illness and intensity of service; coordinating communication with physicians. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the patient served. Must demonstrate knowledge of the principles of growth and development as it relates to the different life cycles. Specific age groups that are served by this position are infant, pediatric, adolescent, adult and adult/geriatric. Responsible for review of the medical record to assure timely and appropriate utilization of hospital resources. Must be able to interact positively with persons of all ages, and to demonstrate the highest level of ethical conduct. Participates in multidisciplinary discharge planning program.
