In this role, you will be a vital member of the patient’s interdisciplinary team guiding them through their cancer care journey.
Position detail: The Nurse Navigator role is a salaried position, working 40 hours per week, Monday-Friday. This position is mostly in-person with some opportunities for remote work (1 day per week), and offers many appealing benefits including health & retirement plans as well as generous PTO accruals.
Summary:
Each patient has an assigned nurse navigator to oversee their diagnosis and treatment, help them get their needs met, and serve as their personal resource. The specific purpose of the nurse navigator is 1) to expedite and facilitate access to initial and ongoing care in our system, 2) to personalize the care our patients receive, 3) to provide a single gateway for our patients to minimize confusion over communication and 4) to provide a consistent contact for our patients from diagnosis to survivorship. The nurse navigator is patient-oriented, not physician or division specific, and will facilitate care of the patient and family across the many disciplines and professionals required for their care. The nurse navigator acts as the liaison for the patient and family addressing their concerns and questions, and works with program assistants to arrange their various appointments with their providers.
Essential Functions: An Oncology Nurse Navigator (ONN) works directly with cancer patients and caregivers to identify barriers facing families that may interfere with care delivery and cancer care outcomes. The Navigator is a key member on the UNC Cancer Care Navigation team (nurse navigators, patient navigators and volunteer patient navigators).
KEY RESPONSIBILITIES: The ONN utilizes clinical nursing skills to provide in-person cancer support visits, virtual contact and proactive outreach serving oncology patients, families, and caregivers. The Nurse Navigator practices oncology navigation that support four areas: education, advocacy, resourcing and support. The Nurse Navigator provides individualized guidance to identify health care barriers and to promote timely assistance to decrease unmet needs and to provide easy access to quality health and psychosocial care. The Nurse Navigator maintains an extensive knowledge of available resources within UNC Health, the North Carolina communities, and national oncology foundations and serves as a clinical resource for this navigation program. The Cancer Care Nurse Navigator will collaborate with the Lead Navigator in the development and expansion of the Cancer Care Navigation Program to UNC affiliates. • Navigates and completes comprehensive barrier assessments in four domains: health promotion (i.e. access to medical care, food insecurity, tobacco cessation and exercise), logistics (i.e. transportation, lodging, parking), financial (i.e. completion of internal and external assistance applications, foundation monies), and emotional and social support (i.e. coping resources, referrals, advance directives, and community engagement). • Navigates patients, families and caregivers through the cancer care continuum providing resources and interventions appropriate for the unmet needs identified in the barrier assessment. • Oversees daily navigation assignments, interventions, data collection, and documentation for the Oncology Patient Navigators • Partners with Lead navigator in the onboarding, orientation, and continuing education of the Patient Navigators. • Collaborates in quality and research efforts for the Cancer Care Navigation Program • Serves as resource/contact for the Patient Navigators and Volunteer Patient Navigators • Serves as nurse liaison with the clinical nurse navigation team. • Collaborates effectively with medical team and hospital staff using professional communication and proper channels • Partners with research coordinators for community outreach involving active surveillance/cancer screening of high-risk patients and participates in research initiatives for the UNC Cancer Care Navigation and the requirements for the Commission on Cancer accreditation. • Communicates effectively with patients, families and caregivers building a trust relationship, mutual respect, and demonstrates empathy, integrity, honesty, compassion and cultural sensitivity in all interactions. • Empowers patients, families and caregivers to self-advocate and communicate well with medical team. • Upholds diversity, equity, and inclusion in all interactions and identifies barriers created through gaps in health literacy and language discordance. • Facilitates patient- & family-centered care that is appropriate/effective for the cancer treatment and the promotion of health. Responsibilities:
1. Communication - Nurse Navigators will ensure that there is continuity of communication between a patient's various care providers (both within and outside the UNCHCS community)

Patient Care Duties Interviews patients regarding their current health practices, identifies health problems, determines the degree of compliance and problems complying with the prescribed treatment plan, and documents this information in the patient's chart. Analyzes data from patient interviews, chart reviews, and physical assessments to determine patient's nursing care needs including direct care, referral to other departments. Additional duties related to coordinating patient care specifically for the cancer patient. Coordinates scheduling of tests labs, x-rays coordinates consults. Interacts with referring physician's offices for new and ongoing patient care issues.

Other information:Education Requirements:● Bachelor's degree in Nursing (BSN) from a state-accredited school of professional nursing.● In lieu of bachelor’s degree, candidate may possess ADN but will be required to enroll in a BSN program within 4 years, and obtained BSN within 7 years.Licensure/Certification Requirements:● Licensed to practice as a Registered Nurse in the state of North Carolina.● Oncology Nursing Certification (ONC) within two (2) years of employment.
