Work Schedule: Monday - Friday, 7:00 AM - 5:00 PM (This position is for 10 hour shifts, 4 days a week)
Accountable for the delivery of quality care to patients and families according to the individualized needs of the patient.
Provides diabetes expert care in the outpatient and inpatient settings.
Responsibilities:
Essential Functions
1. Provides appropriate care specific to the age of the customer/patient, to ensure understanding and comfort level of treatment as outlined in the Population Specific Criteria.
- Demonstrates ability to identify opportunities for and give individualized customer service/patient care according to customer/patient needs.
2. Performs an initial and ongoing assessment to determine the patient's needs.
- Includes data from the patient/significant other(s) in the assessment as appropriate.
- Prioritizes data collection according to patient's immediate needs.
3. Develops/reviews/revises and individualizes the plan of care to address the patient's nursing care needs for optimum patient care outcomes. (Consultation and resources, nurses as teachers.)
- Collaborates with providers and other clinical disciplines as appropriate.
- Provides teaching specific to the patient.
- Care plan reflects continuing care needs and/or referrals.
4. Implements interventions that are consistent with the established plan of care in a safe and appropriate manner (Professional Models of Care).
- Is familiar with, and follows, nursing protocols and standards of care.
- Uses current knowledge in nursing practice based on standards and research literature.
5. Performs technical skills competently and independently to assure patient safety (autonomy).
- Able to competently perform technical skills independently.
- Is accountable to learn new skills and equipment within areas' specifications and time frame.
- Demonstrates correct and safe technique in the use of equipment according to specific product information and policy and procedure manuals.
- Follows appropriate procedure for obtaining and returning or cleaning/disposing of equipment and supplies.

- Completes appropriate report form for a risk (e.g., variance/incident report, patient concern report, maintenance request).
- Communicates to appropriate personnel regarding unresolved issues.
- Works within the scope of practice as defined by Nurse Practice Act.
8. Fiscally accountable.
- Acts professionally responsible for human and other resources.
- Charges accurately for use of supplies and equipment according to policy.
9. Maintains knowledge of current diabetes practice as per American Diabetes Association diabetes self-management education program recognition guidelines.
10. Ensures quality education will be delivered to patients with diabetes who require education.
- Provides individualized diabetes education based on identified patient needs and the nursing process.
11. Effectively collaborates with others to provide quality care to the patient with diabetes.
- Works with staff nurses and food service staff to assess, plan, implement and evaluate patient education of diabetes.
- Collaborates with physicians and other health team members to provide quality care.
12. Participates in the operations of the Center For Diabetes & Nutritional Health to:
- Meet the needs of the hospital for diabetes services.
- Promote the philosophy and marketing plan.
13. Markets the Center to Administration, physicians, hospital personnel, and community.
14. Acts as consultant/resource in all aspects of diabetes care to hospital and community.
- Provides learning opportunities for health care professionals and students regarding diabetes care and education.
- Communicates with other programs and providers to ensure effective use of Health System and community resources.
15. Participates in the development and implementing the performance improvement action plan.
16. Use interpersonal techniques with individuals that are intended to:
- Result in desired objectives/outcomes.
- Be perceived as satisfying/acceptable by those involved.
- Demonstrates a team effort with both employees and physicians.
- Handling customer relations with patients and families including complaints, concerns, and compliments.

Job Requirements
Education
- Bachelors of Science in Nursing (BSN) from an accredited school of nursing required.
- Successful completion of annual mandatory education requirements.
Experience
- Three years clinical nursing experience with progressively more responsibility with clinically challenging patients.
- Minimum of 2 years diabetes education experience; CDE or CDE eligible.
License/Certifications
- Current valid Registered Nurse (RN) License, valid compact multi-state license, or a temporary permit while awaiting licensure required for work state.
- Current American Heart Association or American Red Cross Basic Life Support (BLS) strongly preferred at time of hire, required within 3 months of hire.
- Certification(s) in area of specialty preferred.
- Certified Diabetes Care and Education Specialist (CDCES) through the Association of Diabetes Care & Education Specialists (ADCES) preferred.
