What You Can Expect:
The Nurse Navigator (NN) helps patients navigate the healthcare system, addressing barriers such as housing, jobs, and transportation. The NN conducts pre-visit planning (PVP) appointments before a patient's first visit with the physician to review medications, allergies, health history, care team members, and necessary resources or assistive devices.
Key responsibilities include:
- Escalating patient needs to the care team when significant resources are required.
- Assisting with resources, FMLA/disability paperwork, coordinating meetings, and tracking referrals.
- Collaborating with providers and care teams to ensure accessible care.
- Supporting staff nurses as needed and reviewing referrals for triage.
Scheudle: Monday-Friday 8:00am-5:00pm
Qualifications:
- Education: Bachelors of Nursing Degree
- Experience: 2 years's of RN experience
- Licenses/Certifications: Current RN License in the State of Practice AND Current Basic Life Support (BLS) Provider Certified through American Safety and Health Institute, American Heart Association, or American Red Cross.
