Schedule Details/Additional Information:
Rotating Schedule - Multiple Sites/Locations.
Supporting the Illinois Masonic (IMMC) Region - Endocrinology Department and Physicians.
M-F, No Weekends or Holidays.
Major Responsibilities:
- Patient Education/Teaching (Continuous Glucose Monitors, Medication Administration and Injections, Glucometer Teachings)
- Endocrinology Triage – perform telephonic or in person triage related to endocrinology specific symptomatic complaints
- Collaborate with the Endocrinologist and Medical Assistant to address and escalate any urgent care related needs
- Delegation of any tasks to the medical assistant
- Prior Authorizations
- Durable Medical Equipment – point of contact for products/DME vendors to check on status, expediate and coordinate care
- Population Health Outreach – Trend A1c Testing and control and reach out to patients who are overdue
Licensure:
Education/Experience Required:
- Graduate of an accredited school of nursing (BSN Preferred)
- BLS Certified – Healthcare Provider
- 2 years of RN experience preferred
- EPIC experience preferred
Physical Requirements and Working Conditions:
- Ability to work weekends, holidays, and different shifts to accommodate staffing
- Certain positions may need to take call
- Ability to transport objects, patients, and equipment
- Ability to respond quickly to patient needs and work at fast pace
- Work environment includes probability of exposure to adverse, hazardous, or unpleasant conditions while caring for the sick
- Ability to assist in evacuation of patients in the event of a fire or other disaster
- Routinely works with potentially infectious specimens and patients
- Ability to respond to patients in a timely manner and access all areas of the unit
- This requirement supersedes any previous lifting requirement effective 04/20/2014
- Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
