Description
1. Demonstrates and promotes excellence in the application of the AC Model of Patient and Family-Centered care. Collaborates and role models partnerships with patients, families and other providers to plan, deliver, and evaluate highly individualized care that ensures achievement of effective self-care by patients and families. Consistently plans and delivers high quality, comprehensive and holistic care for complex patient situations.
