POSITION SUMMARY:

The Stroke Coordinator, under the direction of the Clinical Quality Director/Medical Director, is a licensed professional nurse that works collaboratively with the health care team, is customer focused and strives to meet unit and hospital goals in order to promote quality care for the stroke patients. The stroke coordinator is knowledgeable on comprehensive stroke program standards and guidelines. The stroke coordinator keeps all aspects of patient care as it relates to stroke coordinated following program standards.

KEY RESPONSIBILITIES:

• Reviews current program standards related to certification

• Updates on changes that affect the stroke program (guidelines, stroke care

evidence orders, processes-ex: 2018 ischemic stroke guidelines).

• Concurrent chart review on all stroke cases to identify opportunities for improvement according to latest AHA/TJC standards.

• Maintain ongoing daily stroke patient list for review while maintaining highest level of following all required stroke indicators with multidisciplinary team.

• Collaborates with Neuroscience APRN in developing outcome driven PI indicators

• Attend ED and IP code strokes

• Point of contact in healthcare system for stroke program

• Community Education/Outreach on Stroke following AHA guidelines

• Staff Education on Stroke (Nursing and Medical Staff) using AHA guidelines using different methods of instruction.

• Attends General Orientation to educate all new staff with stroke and the Comprehensive Stroke Program.

• Attends Nursing orientation to educate all new nurses on the Comprehensive Stroke Program nursing responsibilities.

• Attends regional meetings on Stroke

• Data entry- GWTG

• Attends stroke program meetings

• Attends core team meetings

• Collaborates with interdisciplinary team when opportunities are identified in an effort to correct issues.

• Provides feedback to the staff on new measures and initiatives related to the stroke program, provides staff education to assure compliance.

• Active participant of the stroke collaborative, AHA get with the guidelines and other community-based stroke groups.

• Active reporting member of the Comprehensive Stroke Committee meeting in conjunction with Stroke medical director

• Maintains survey readiness at all times.

• Complies with regulatory readiness at all times by providing required documents and attending required training.

• Assists with TJC surveys as required

• Assists with AHCA surveys as required

REQUIRED KNOWLEDGE & SKILLS:

(Examples: Ability to work independently and take initiative; Good judgment and problem-solving skills; Communication skills; Interpersonal and organizational skills; Level of confidentiality)

• Five or more years of clinical experience in ED, Critical Care, or Neuro ICU Unit.

• Basic computer knowledge.

• Knowledge of general nursing theories and practice.

• Experience in data collection, entry and analysis required.

• Experience with AHA/TJC indicators and standards

• Graduate from an approved nursing educational program as defined by the National Council of State Boards of Nursing (NCSBN)

EDUCATION/EXPERIENCE/LICENSURE/TECHNICAL/OTHER:

I. Education: Registered Nurse Licensure in the State of Florida

II. Experience: Minimum 5 years’ experience in Critical Care ED,

III. Software/Hardware: Knowledgeable in Word, Excel, and PP

IV. License/Certification: Other: BLS/ACLS certifications
