POSITION SUMMARY

The MDS coordinator (Minimal Data Set) is responsible for gathering information on a healthcare facility's current and future residents for future and current assessment, including physical and mental states. MDS coordinators assess charts and communicate with health care teams to create applicable health care plans for their current and incoming residents. They are ultimately responsible for adequately assessing residents’ needs, and coordinating personalized, resident-driven care based on those assessments. They work with nursing, rehab, nutrition, and other key disciplines to ensure care plan guidelines for each resident are followed, and compliant with ethical standards and Medicare requirements. The MDS Coordinator facilitates the gathering and submission of via the Resident Assessment Instrument (RAI) process, which assesses a resident’s behavioral, psychological, clinical, nutritional, and physical strengths and needs to create an individualized cared plan that considers the resident holistically.

KNOWLEDGE, SKILLS & ABILITIES

Current RN License in State of Georgia or Multistate Required. One (1) year experience in related specialty area preferred. Ability to work well individually and as a team leader or member. Demonstrated ability to assess and adapt to new situations, think critically, manage priorities and utilize problem-solving techniques. Ability to read and interpret policies, standards, and think critically evaluating patient care issues. Strong clinical nursing skills in assessment, planning, intervention and evaluation. Ability to communicate clearly with, and care for diverse population of patients/family units of all ages, a variety of cultures, and/or persons occasionally exhibiting stressed behaviors. Ability to operate clinical equipment, computer, telecommunication devices and patient information systems.

WORKING CONDITIONS, ADA INFORMATION

Works primarily in skilled nursing facility setting. Requires considerable standing, mental effort, stooping, bending, lifting (in excess of fifty (50) lbs.), pushing objects (carts, beds, and wheelchairs). Likely to be exposed to blood and body fluids, including infectious diseases. May rarely require accompanying and assisting with patient transport to another facility. Uses medical, office equipment, and telecommunicates devices on a regular basis (e.g., I.V. pumps, fax, computer, and pagers). Physical functional capacity for healthcare workers as defined by the Saunders Method (very heavy).
