Villa St. Joseph was founded in 1997 as a ministry of the Sisters of St. Joseph. 20 hour/week. Every 6th weekend rotation

Position Summary:

The Registered Nurse Assessment Coordinator (RNAC)or Licensed Nurse Assessment Coordinator (LNAC) is an RN or LPN who is responsible for the overall supervision, coordination, maintenance and timely completion of the MDS, care plans and medical records for residents.

Qualifications:

- Registered Nurse (RN) or Licensed Practical Nurse (LPN) currently licensed by the state of PA

- Two (2) years nursing experience

- RNAC/LNAC certification

- Possess effective communication and interpersonal skills, and excellent organizational and problem-solving skills

- Must possess working knowledge of federal, state, local regulations, policies and procedures, Medicare and Medicaid reimbursement guidelines and procedures as they apply to resident care plans and reporting requirements

- Must possess CPR and First Aid certificates

- Must be able to operate a personal computer

Essential Duties and Responsibilities:

1. Oversees and coordinates assessments of resident needs through completion of CAAs, MDS, and quarterly reviews

2. Meet routinely with Unit Manager, Charge Nurses, and Nurse Aides through daily rounds and attends other interdisciplinary team meetings to note any change in resident status and to assure residents’ plans of care are being correctly implemented

3. Determines level of care needed and submits required forms and reports to Medicare and Medicaid

4. Reviews Medicare eligible residents and assures orders and qualifications meet Medicare guidelines

5. Provides staff training and implements efficient tracking procedures to assure all nursing staff are familiar with identifying and accurately documenting and reporting any changes in resident condition.

6. Ensures that documentation clearly supports the assessment and validates an accurate level of care is being provided

7. Works on-call to assist nursing staff with resident care.

8. Commits to high ethical standards and complies with the Code of Conduct, state and federal regulations, accreditation standards and Concordia policies and procedures.Maintains confidentiality of resident and employee records and information.Has a duty to report compliance concerns, significant legal risk questions, fraud, abuse, suspected or actual misconduct to a supervisor/manager or to the Corporate Compliance Officer.
