Overview
Works under the supervision of the office manager, RN or clinical lead, and the physicians. Responsible for assessing, planning, implementing, and evaluating total nursing care of assigned patients. The CMA/LPN may work in a variety of settings and roles within Riverside Medical Group providing hands-on clinical care to include manual vitals, medication review, injections and blood draw. The CMA/LPN utilizes communication, critical thinking, problem solving, and decision making to create a care plan for each patient based on physiological, psychosocial and cultural diversity. The CMA/LPN may be responsible for supervising and delegating non-professional personnel working directly with him/her to ensure optimum patient care. Must demonstrate competence in dealing with age related needs of specific patient populations served in their assigned unit/dept. Works under the direction of leadership and a multi-disciplinary team to provide a comprehensive range of services or clinical care associated with and in accordance with standards of excellence established by Riverside Health system and facility goals and strategic plans.
What you will do
- Accurately completes all required clinical documentation consistently per RHS standards. Maintains documentation for quality assurance to include but not limited to point of care testing, medication logs and annual wellness visits. Works with providers to assure care gaps are routinely addressed with each patient. Documentation is accurate, legible and completed same day. Knowledgeable and skillful in lab functions. Monitors OSHA and lab proficiency testing. Maintains clean, compliant, well-stocked work areas.
- Provides patient education as needed. Teaches patients as necessary about the medications prescribed for them. Demonstrates positive patient care by explaining the importance of the completion of registration and patient medical history forms and how it relates to their care at our facility. Provides patient follow up with a sense of urgency regarding lab calls, patient phone calls, prescription fills and refills, referrals and appointment scheduling.
- Ensures proper reimbursement from insurance companies by performing pre-authorization for prescription drugs, procedures, and patient tests.
- Demonstrates the ability to build trusting collaborative relationships with a multi-disciplinary team to support patient care needs. Actively participates in multi-disciplinary team meetings as assigned.
- Answer telephone calls from patients, pharmacies, physicians, vendors, etc. by close of business. Communicate lab/pathology results and schedules additional tests per provider order.
Qualifications
Education
- Licensed Practical Nurse (Required)
- Bachelors Degree, Nursing (Preferred)
Experience
- 1-3 years Nursing experience (Required)
Licenses and Certifications
- Licensed Practical Nurse (LPN) - DHP Health Professional Data Services (Required)
- CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) (Required)
