The Licensed Practical Nurse is responsible for administering medications, conducting nursing assessments, maintaining accurate records, monitoring medication inventories, and communicating effectively with clients and staff. Key Responsibilities:
- Administer medications to clients via various methods, ensuring proper dosage and adherence to protocols.
- Conduct comprehensive nursing assessments, including physical, mental health, and psychiatric evaluations.
- Maintain accurate and timely documentation of medication administration, assessments, and client interactions.
- Monitor medication inventories, ensuring adequate stock and timely refills for residential clients.
- Communicate effectively with clients, addressing medication queries and coordinating refill requests with providers.
- Verify client data, medical history, and visit purposes, facilitating seamless care delivery.
- Participate in nursing-led educational groups and contribute to client safety planning.
- Assist with general lab duties and phlebotomy as needed.
- Serve as a resource for nursing and medical issues, promoting client hygiene and infection control measures.
- Actively participate in required meetings and perform other assigned duties as delegated by nursing leadership.
Education and/or Experience Qualifications:
- Current licensure as a Licensed Practical Nurse (LPN) in the state of practice.
- Graduation from an accredited practical nurse program.
Required License/Certification (one of the following):
- Licensed Practical Nurse (LPN)
Additional Qualifications:
- Successful completion of background check including criminal record, driving record, and abuse/neglect.
- Valid driver's license in state of residency with an acceptable driving record.
- Strong communication skills, both verbal and written, with the ability to produce clear and concise reports.
- Proficient in accurately entering, transcribing, recording, and storing information in various formats.
- Skilled at reading and interpreting various documents, including laboratory results and electronic health records.
- Competent in basic mathematical calculations.
- Possesses strong problem-solving abilities and adaptability.
- Demonstrates a willingness to learn, grow, and work collaboratively with diverse perspectives.
