Job Description

Position Summary: The LPN-Triage is responsible for providing telephone triage, advice and health education within the protocol guidelines established for NJU.

This role is 100% on site.

Essential Job functions:

Answers patient calls on a wide range of topic, asks the right questions to get the information needed to make an accurate assessment
Extreme listening skills with the ability to interpret subtle indications as tone of voice, hesitations or incomplete responses.
Ability to assess the problem, based upon the conversation, and use clinical judgment, knowledge and experience to direct the conversation and guide in the decision to give self-care advice, refer the patient to the primary care physician, make an appointment or call 911 for emergency assistance
Understands and uses appropriate triage resource for protocols, as identified by Summit Medical Group
Acts as a patient advocate and upholds patient rights
Documents all phone encounter into the Electronic Health Record
In depth knowledge and understanding of disease processes, drug reactions and preventive care
Rapid and effective transmission of patient data between all relevant parties
Provides patient education when appropriate

General Job functions:

Assists with orientation of new personnel
Attends staff meetings
Participates in mandatory education at Summit Health Management
Other duties as assigned

Physical Job Requirements:

Dexterity of hands and fingers
Endurance (continuous typing)
Sitting for long periods of time
Concentration and memory – complex and time pressured decision making with a high degree of accuracy
Multi-tasking, organizing and priority setting

Education, Certification, Computer and Training Requirements:

Vocational / Technical School / Diploma Program required. Associates degree preferred
New Jersey State Nursing License and BLS certification required.
0 – 1 years experience. 2-4 years experience preferred.
Ability to communicate in English, both orally and in writing.
Strong interpersonal and organizational skills.
Ability to prioritize and organize multiple tasks in a timely manner.
Experience with Standard Office Equipment (Phone, Fax, Copy Machine, Scanner, Email/Voice Mail) preferred.
Experience with Standard Office Technology in a Window based environment preferred.
Experience with patient care equipment preferred.
Experience with EHR, Practice Management System and Departmental/Ancillary Systems preferred.

Travel: Ability to commute to satellite offices as needed.
