The RSFH Hospital at Home program delivers acute hospital-level care to patients in the comfort of their own homes. Patients meeting inpatient admit criteria for qualifying medical conditions, such as cellulitis or pneumonia, will be admitted into the Hospital at Home program upon successful completion of a program eligibility assessment. A professional and neat appearance is required to project a positive first image of the Roper Saint Francis Healthcare system. Ability to maintain strict confidentiality of work-related information. Contacts: Constant interaction with internal and external customers including (but not limited to) patients, families, physicians, and coworkers. For Virtual Registered Nurses working outside of the hospital setting (i.e. Hospital at Home), the applicant must have a reliable internet connection and dedicated workspace with the ability to lock up patient information. Summary of Primary Function Delivers patient care utilizing the nursing process through data collection and assessment, identifies and determines the priority of patient’s problems/needs, and evaluates the process and outcome of nursing care; coordinates, provides and directs patient teaching activities and coordinates the care provided by health team members. Essential Job Functions In collaboration with the interdisciplinary care team, conducts initial and ongoing patient assessment, analyzes assessment data, creates a plan of care, implements treatment and evaluates treatment effectiveness; administers medications in a safe manner consistent with the State of Practice and organization policies and procedures; serves as point of contact for patients' care coordination throughout hospital departments. Acts as a patient safety advocate by participating in ongoing quality improvement in the department.
