Job Summary and Qualifications
Facilitates the development, implementation and ongoing adherence of sepsis program protocols and therapies. Ensures protocols and therapies become the standard of care followed within the facility. Collaborates with medical and hospital staff to provide multidisciplinary care to patients with a diagnosis of sepsis. Rounds with Physicians with diagnosis of sepsis, reviews clinical data. Provides education and clinical expertise to care team, patients, and families. Collaborates with physicians and staff in an effort to decrease mortality and length of stay. Identifies and integrates patient learning needs, abilities and readiness into the Plan of Care. Works in collaboration with the nursing team to develop and implement individualized teaching plans for patients and families. Collects and analyzes sepsis program patient data to assist with documentation, clinical care, and improved outcomes. Participates in rounding and proactively responds to patient needs to improve patient outcomes and positively impact overall patient experience. Contributes to a culture of patient centered care by ensuring a quiet healing environment.

Demonstrates knowledge by successfully completes required hospital competencies regarding medications/drips, interpretation of EKG strips, ABGs, disease processes, treatments and nursing assessment and interventions. Demonstrates quality by documenting the application of the nursing process in a responsible, accountable and ethical manner.
- Demonstrates understanding of patient’s hemodynamic status and reports critical changes in condition to the physician. Documents the communication in Physician contact, nursing notes. Develops the plan and establishes the plan priorities with the patients, family and others as appropriate.
- Documents the daily plan of care /goals for the patient in Meditech and white board in patient room. Engages in team work as a team player and a team builder.
- Follows the appropriate policies or processes for acknowledging physician orders, initiation of approved protocols, transferring or discharge processes.
- Patients going for procedures or OR.
- Completes the appropriate documentation in Meditech and assures the proper paperwork follows the patient. Follows the appropriate process for bed management, patient throughput by assuring effectiveness, positive patient outcome with high quality care and high patient satisfaction.
- Follows the Standards of care for Critical care regarding assessment, data collection, plan of care, documentation.
- Shows high competence of using Meditech.
- Able to navigate with confidence and retrieve patient care information. Follows through with assignments and informs supervisor if unable to complete task.
- Follows with the unit/hospital initiatives throughout the year and assures consistency, self-accountability in executing. Interacts with peers and colleagues to provide feedback and enhance ones own professional nursing practice and/or role performance. Involves the patient, family, and other healthcare providers in formulating expected outcomes when possible and appropriate. Participates in ongoing education activities related to appropriate knowledge bases and professional issues yearly competencies, evaluation education requirements and etc.
- Participates in quality improvement activity, such as but not limited to collecting data; to monitor quality and effectiveness of nursing practice; analyzing the data to identify opportunities for improvement; formulating recommendation to improve nursing practice or outcomes.
- Provides effective hand off communication with other departments, at change of shift, during assignment coverage. Shows knowledge and understanding of shift change rounds and hourly rounds. Able to document the above as required. Provides health teaching that addresses such topics as risk reducing behaviors, developmental needs, preventive self-care, activities of daily leaving. Documents in Meditech.

- Uses health teaching methods appropriate to the situation and the patients developmental level, learning needs, readiness, language preference, culture.
- Seeks opportunities for feedback and evaluation of the effectiveness of the strategies used. Documents the education provided as required in Meditech.
What qualifications you will need:
- Current Florida license as Registered Nurse or Recognized Compact Licensure (subject to Florida State Licensing Requirements, including/not limited to ongoing eligibility and duration provisions)
- Graduate from an accredited school of nursing, Bachelors degree in nursing preferred.
- Current Basic Life Support Certification through the American Heart Association Current ACLS certification through the American Heart Association.
- Minimum of 2 years of clinical experience preferred.
- Completion of Basic Arrhythmia course with a minimum of 16 hours of didactic instruction preferred; Critical Care Course preferred.
- Certification as CCRN preferred.
