Registered Nurse (RN) - Home Care Case Manager
Bristol Home Care and Hospice Agency, Inc. is seeking a FT RN Case Manager. Hours are M-F, 8-4:30 with some on-call requirements. Bristol Home Care is a 5-star patient satisfaction rated home care agency serving Bristol and the surrounding towns. Caseloads for FT RNs is 20-25 patients. Our smaller territory of the Greater Bristol area decreases driving time and increases the time you can spend with your patients. Key functions of the RN Case Manager include:
-Evaluates patients referred for home care to determine appropriateness for admission. Refers patients needing other services to appropriate agencies.
-Consults with family, institutions, physicians, patients, nurses, and others, while establishing a coordinated home health care plan.
-Completes OASIS nursing assessment, written plan of care (485), and completed assessment.
-Reviews nursing care plans/complete OASIS requirements and update as required.
-Reviews physician's plan of treatment, orders, and insures execution.
-Completes a written report to physician as needed, with Plan of Treatment summary.
-Provides supervision, evaluation, guidance and counseling to all home health staff
-Current RN, Registered Nurse License in the State of Connecticut

Educational / Minimum Requirements:
State/Federal Mandated Licensure or Certification Requirements:
General orientation at time of hire. Fire/Safety/Infection Control annually. Health Care Provider CPR from American Heart Association or Professional Rescuer CPR by American Red Cross or other similar programs deemed appropriate by management. Successfully complete all training programs as determined by Education & HRD department. Other unit based competencies/skills as required.
Special Requirements:
Successfully complete unit based competency package, plus unit specific items. Strong team skills.
For Home Care Agency:
Must have a car to drive to home visits and a valid Connecticut driver’s license in good standing; $100,000-$300,000 automobile liability insurance.
Physical Requirements:
Physical agility with repetitive occurrence including but not limited to; walking, running, bending, stooping, climbing stairs and ladders, kneeling, crouching, crawling, balancing, reaching, standing, sitting, and twisting. Lifting greater than 50 pounds repeatedly throughout the work day. Looking at a computer monitor. Vision acuity correctable to normal. Hearing acuity correctable to normal. Acute sense of smell for normal perception. Ability to verbalize so that the average person can comprehend.
Work Environment:
Regular exposure to patient elements, occasional stress.
