Position Summary: Reporting to the Manager, with direction from the Registered Nurse, this position is responsible for providing high quality, individualized basic care activities as identified by the RN, using the components of the nursing process. - Performing duties in a manner that provides for the physical, psycho-social and spiritual needs of the patient including performing basic patient care activities. Incorporates cultural and ethnic factors into patient care activities within the limitation of the job, correctly differentiating and reporting normal and abnormal findings with direction from the RN and incorporates the functional/developmental age into these activities for the adolescent through geriatric patient population.
- Using hospital and regulatory standards of care, the CNA demonstrates sound judgment in the implementation and basic nursing assistant aspects of the trans-disciplinary care plan. In conjunction with the patient/family and with the direction of the RN, the CNA provides care based on the individualized patient care plan. The CNA observes and reports patient issues/concerns/problems and on an ongoing basis will report findings to LVN or RN.
- Contributing to the patient care process by assisting the patient in basic living activities such as bathing, oral care, positioning, line changes and ambulating.
- Responding appropriately to concerns regarding patient safety based on the physical environment, equipment and the patient’s behavior.
- Demonstrating the ability to provide care in emergency situations and following established emergency procedures and practices, as appropriate.
- Communicating and following the organizational chain of command for notification of patient care/service issues, when appropriate.
- Maintaining knowledge of, and demonstrating the ability to identify patient care issues and assisting with patient care activities for the adolescent through geriatric patient population. Assisting with appropriate care through the healthcare continuum.
- Assuring that the rights of all patients are respected and maintained by allowing for privacy and dignity in the provision of care activities.
- Being aware of the reporting responsibilities to, and reporting suspected cases of abuse or neglect relative to the adolescent through geriatric patient population as appropriate.
- Reinforcing patient/family educational needs as identified by the RN and helping provide individualized basic teaching during the care giving process which considers the functional/developmental age and the needs of the adolescent through geriatric patient population. Observing and reporting the effectiveness of the patient/family teaching through observation of patient responses.
- Demonstrating knowledge of age specific care for the adolescent through geriatric population, including but not limited to identifying the need for additional safety measures, physiological normal values/readings; and assessing skin integrity, behavior, motor skills and/or activities that place patients at risk as well as communicates effectively in a clear, concise, understandable manner.

- Practicing Universal Precautions.
- Ensures completion of hourly rounding and updates white boards in a timely manner.
- Completes departmental and hospital-wide in-services, competencies and Health Stream assignments in a timely fashion
- Attends staff meetings and actively participates, reads and returns all staff meeting minutes.

- Demonstrates an understanding of the procedures for reporting and responding to facility emergencies such as fire, and disruption in utilities and hazardous materials spills
