Position Overview:
The Clinic RN is responsible for the care of clinic patients. The Clinic RN is skilled in all aspects of nursing process, delegates and supervises staff in assistive roles on the same shift and is responsible to deliver the highest of customer service standards for patients and families presenting for services at Winona Health. The Clinic RN will assess and triage patients to make certain of the appropriateness of care. Will provide patient care including but not limited to rooming of patients, performing assessments and gathering of data for the physician. Patient information will be entered into the Electronic Medical Record (EMR).
Responsible to provide direct patient care and assist in the development and coordination of Winona Health’s wound care program. Establishes standards of care and clinical competencies specific to the prevention, care and management of wounds, and develops and provides training programs to ensure clinical expertise across all of Winona Health Services. Coordinates and collaborates with other staff making recommendations for patient care that are evidence based, cost effective and individualized. Coordinates and directs Wound, Ostomy and Continence care across all of Winona Health.
Essential Duties & Responsibilities:
- Plans and provides care to patients with pressure, venous, arterial, diabetic, and cancerous and other types of ulcers.
- Collaborates with nursing and providers in assessing patient and family specific needs and develops a plan of care of complex high risk wound patients. Evaluates the effectiveness of the wound care provided.
- Provides care in collaboration with members of the multidisciplinary team, in compliance with policies and procedures, standards of care, and regulatory requirements. Participates in the development and implementation of procedures and protocols based on best available evidence related to the care of wounds, ostomies, and incontinence.
- Introduces new techniques/innovations based on advanced wound care management, practice and trends.
- Leads continuous systems improvement (CSI) projects that successfully reduce the percentage of patients who develop pressure ulcers, wound complications and continence issues.
- Collects and analyzes data on wound incidents and makes recommendations for continuous improvement.
- Provides education and counseling to patients and families about preventative measures and care techniques to optimize wound healing.
- Provides education, consultation, treatment options, and direct assistance to staff and providers related to wound prevention, evaluation, and management.
- Provides ongoing education for staff maintaining current knowledge and competency in the areas of wound and skin management, and ostomy and continence care.
- Initiates and reviews/revises the plan of care to reflect changing patient needs based on the evaluation of the patient’s status.
- Promotes continuity of care by accurately and completely communicating to other caregivers the wound status of patients for whom care is provided.
- Recommends standards for selection of cost-effective supplies and equipment for wound, ostomy, and continence based on best evidence, product evaluations, and cost-effectiveness comparisons.
- Documents assessment, planning, implementation and evaluation in the patient’s electronic medical record.
- Documentation reflects objective/subjective data, interventions and patient’s response to treatment.
- Performs wound consults in ER, Acute, Long-term Care, Home care/Hospice Dialysis, and Clinics.
- Selects optimal stoma sites pre-operatively to promote post-operative independence.
- Performs foot care for patients in all settings as necessary.
- Ensuring nursing supplies and medications are properly ordered and available.
- Other duties as assigned.
Essential Skills and Experience:
- Bachelors of Science Degree in Nursing.
- Current Registered Nurse license in the state of Minnesota.
- Certification as a Certified Wound Care Nurse (CWCN) or CWOCN (Certified Wound Ostomy Continence Nurse). CWOCN required within 18 months of employment.
- BLS certified.
Nonessential Skills and Experience:
Minimum of 5 years of related clinical experience is preferred.
