RN3 - PRE ANESTH, REGISTERED NURSE - 3135

SUMMARY

Coordinate and develop a plan of care for specific pre-anesthesia patients under the direction of the Anesthesiologist and in collaboration with other members of the health care team. Assists with the pre-anesthesia physical assessment, data collection, and patient teaching. Follows established nursing procedures and practices utilizing ASPAN and ASA Standards. Assembles pertinent patient information into chart format.

JOB DUTIES

Evaluate patient pre-surgical needs including recognition of those patients at risk for cardiovascular, pulmonary and or renal complications.

Obtain accurate nursing history. Relay relevant findings to anesthesiologist.

Demonstrates knowledge of ASPAN and ASA Standards, MMC Administrative and Nursing Policies and Procedures.

Assists anesthesiologist with patient exam.

Implements appropriate orders. Makes arrangements for consults, special patient care needs.

Demonstrates responsibility for containment of hospital costs.

Patient chart is assembled and labeled correctly with history and physical, surgical consent, preoperative test results.

Assumes personal responsibility for professional development and continuing education.

Maintains skills needed to complete preoperative assessment and care for patients of all ages.

Chart review reveals accurate preoperative assessment completed on patients with ages ranging from infant to geriatric.

AGE/DEPARTMENT SPECIFIC COMPETENCIES

Infant - Recognizes this age group as 0-12 months with a mental state of trust vs mistrust.

Recognizes and reports finding outside the normal range for this age group: 1. Lab Values 2. Vital Signs

Assesses pain and documents findings utilizing the FLACC or CRIES scale.

Includes parent/ family member in assessment, exam and development of patient’s plan of preoperative care.

Demonstrates safety measures when caring for the infant patient.

Is aware of the responsibility and mechanism to report abuse specific to this age group.

Positions infant on exam table appropriately.

Pediatric Patient - Recognizes this age group as the following: 1. Toddler (1-3 years of age) with a mental state of autonomy vs shame and doubt 2. Preschool (3-5 years of age) in a mental stage of Initiative vs guilt 3. School age (6-12 years of age)in a mental state of industry vs inferiority.

Recognizes and reports finding outside the normal range for this age group: 1. Lab Values 2. Vital Signs

Assesses pain and documents findings using the FLACC or CRIES scale. May also use FACES scale of 0 –10/10 scale.

Positions patient appropriately, based on physical and mental development.

Demonstrates effective communication when managing the pediatric patient by utilizing pictures and visual aids as a form of communication.

Allows the child to have a security object when possible.

Includes parent / family member in assessment, exam and development of patient’s plan of preoperative care.

Demonstrates safety measures when caring for the pediatric patient.

Is aware of the responsibilities and mechanisms to report abuse specific to the pediatric group.

Adolescent - Recognizes this age group as 13-18 years of age in a mental stage of identity vs role confusion.

Recognizes and reports findings outside the normal range for this age group: 1. Lab Values 2. Vital Signs

Assesses pain by utilizing the 0-10/10 scale, or if non-verbal, the MMC non-verbal pain scale.

Demonstrates effective communication tools when managing the adolescent patient.

Involves both the patient and the parent/family member in developing the patient’s preoperative plan of care.

Is aware of the responsibility and mechanisms to report abuse specific to this age group.

Addresses issues such as alcohol, seat belts and other safety issues for this group as part of incidental teaching.

Adult - Recognizes this age group as 18-64 years of age with the following mental stages: 1. 18-24 years of age - intimacy vs isolation 2. 25-64 years of age - generativity vs stagnation

Recognizes and reports findings that are outside the normal range for the age group. 1. Lab Values 2. Vital Signs

Assesses patient’s pain using the 0-10/10 pain scale or MMC’s nonverbal pain scale, as appropriate.

Demonstrates effective communication tools when managing the adult patient. Involves family member according to patient’s request.

Demonstrates safety measures when caring for the adult patient. Aware of responsibility and mechanism to report abuse.

Geriatric - Recognizes this age group as 65+ years of age in a mental stage of ego integrity vs despair.

Recognizes and reports findings outside the normal range for this age group 1. Lab Values 2. Vital Signs

Involves both the patient (and caregiver if appropriate) in development of preoperative plan of care.

Offers community resources and support help for the patient and family.

Is aware of the responsibility and mechanisms to report abuse specific to this age group.

SPECIFIC JOB DEMANDS

Strength: Medium Work - Lifting, Carrying, Pushing, Pulling 20 - 50 Lbs. occasionally, 10 - 25 Lbs. frequently or up to 10 Lbs. constantly.

Stooping: Occasionally - Bending body downward and forward by bending spine at the waist, requiring full use of the lower extremities and back muscles.

Reaching: Frequently - Extending hand(s) or arm(s) in any direction.

Handling: Frequently - Seizing, holding, grasping, turning, or otherwise working with hand or hands. Fingers are involved only to the extent that they are an extension of the hand, such as to turn a switch or shift automobile gears.

Fingering: Frequently - Picking, pinching, or otherwise working primarily with fingers rather than with the whole hand or arm as in handling.

Feeling: Frequently - Noting attributes of objects, such as size, shape, temperature, or texture, by touching with skin, particularly that of fingertips.

Talking: Frequently - Expressing or exchanging ideas by means of the spoken word to impart oral information to clients or to the public and to convey detailed spoken instructions to other workers accurately, loudly, or quickly.

Hearing: Frequently - Perceiving the nature of sounds by ear.

Near Acuity: Frequently - Clarity of vision at 20 inches or less.

Accommodation: Occasionally - Adjustment of lens of eye to bring an object into sharp focus. This factor is required when doing near point work at varying distances from the eye.

Color Vision: Frequently - Ability to identify and distinguish colors.

MINIMUM EDUCATION, KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED

Professional knowledge of nursing theory and practice at a level normally acquired through completion of an accredited School of Nursing.

Currently Pennsylvania State Nursing License.

Working knowledge of ASPAN and ASA Standards at completion of orientation.

BLS required.

Recredentialing of Unit Specific skills annually.

WORKING CONDITIONS

Normal patient care environment with little exposure to excessive noise, dust, and temperature changes.

A large part of work time is spend standing.

Subject to certain distasteful patient care activities and exposure to communicable diseases, and other illness or diseases.

May have exposure to potential hazards such as chemical wastes, X-ray, disinfectants and other hazardous products.
