Description

Curana Health is a provider of value-based primary care services for the senior living industry, including skilled nursing facilities, assisted & independent living communities, Memory Care units, and affordable senior housing sites. Our 1,000+ clinicians serve more than 1,500 senior living community partners across 33 states, and Curana participates in various innovative CMS programs (including owned-and-operated Accountable Care Organizations and Medicare Advantage plans). With rapid year-over-year growth since our founding in 2021, Curana is setting a new standard in innovative care delivery for seniors with high-risk, complex clinical needs, many of whom have been historically underserved by the health care system. Our mission: To radically improve the health, happiness and dignity of senior living residents.

Responsibilities include:

Round and manage clinical care for geriatric patients in partner facilities, including skilled nursing, long term care, assisted living, independent living and memory care facilities.
Travel to facilities to provide medical coverage 4 days/week.
Build relationships with the facility staff, administrator and nursing director.
Engage with the multi-disciplinary care team to address quality of care and patient care needs.
Complete timely documentation and billing for services provided per Curana policy.
Respond to daytime calls from the facility when not physically present.
Assist with the transition to the permanent Curana provider(s).
Serve as a model and representative of Curana Health at all times.

Qualifications:

NP degree
Unrestricted licensure in multiple states with ability to obtain additional licenses within first 30 days
Easy access to a major airport for frequent travel
Compassionate, Intelligent, Motivated, Team Player
