The LPN will:
- Participates in data collection related to patient status and needs, as appropriate to the ages of the patients, growth and development through the life cycle (Neonates, Pediatrics, Adolescents, Adults, Geriatrics).
- Interviews patient on admission and gathers identifying data and health history.
- Observes and reports emergency information to the Physician immediately and on an ongoing basis throughout the visit.
- Documents all pertinent information in the medical record.
- Provides direct patient care within scope of practice to ensure implementation of the Plan of Care.
- Consistently performs all nursing interventions on assigned patients, unless exceptions are discussed with the Physician.
- Consistently and willingly assists other team members in providing care to patients.
- Implements care considering growth and development factors appropriate to the age of the patient (Neonates, Pediatrics, Adolescents, Adults, Geriatrics).
- Informs and instructs patient/significant others on patient’s health/illness state and Discharge Instructions.
- Communicates all routines and health-related procedures performed to patient/significant others, as appropriate to the ages of the patients growth and development through the life cycle factors (Neonates, Pediatrics, Adolescents, Adults, Geriatrics).
- Documents patient/significant other’s response to patient teaching.
- Documents all aspects of patient care.
- Consistently documents all aspects of patient care in a clear, concise and legible manner.
- Consistently documents information in a chronological order at time of occurrence.
- Is knowledgeable of and initiates emergency procedures.
- Initiates and carries out appropriate interventions in the event of a cardiopulmonary arrest, as appropriate to the ages of the patients growth and development through the life cycle factors (Neonates, Pediatrics, Adolescents, Adults, Geriatrics).
- Verbalizes knowledge of location and use of emergency equipment and disaster plan.
- Maintains supplies and equipment.
- Checks emergency equipment and controlled substance records in accordance with established policies.
- Consistently reports faulty/missing equipment upon discovery and replaces when indicated.
- Administers medications in accordance with established Saint Peter’s Healthcare System guidelines.
- Administers correct medications/dosage/route to identified patient.
- Reports medication errors as soon as discovered.
- Monitors and documents adverse reactions to medications.
- Complies with controlled substance records in accordance with established policy.
- Demonstrates knowledge/purpose of medication.
- Documents medication administration appropriately in the medical record.
- Administers medications considering safety and effectiveness of medications relative to the patient’s age related factors (Neonates, Pediatrics, Adolescents, Adults, Geriatrics).
- Demonstrates competency in performing all procedures within scope of practice and requirements for position, as appropriate to the ages of the patients growth and development through the life cycle factors (Neonates, Pediatrics, Adolescents, Adults, Geriatrics), and an understanding of the range of treatment needed by these patients.
- Accurately takes and records vital signs.
- Accurately obtains and documents weights per Physician order
- Changes dressing in accordance with established procedure unless instructed otherwise
- Obtains and tests specimens within shift as directed in accordance with established procedure.
- Accurately measures and records intake and output in accordance with established procedures.
- Performs all procedures outlined in Clinical Procedure Manual within area of responsibility in accordance with established specifications within specified timeframes.
- Seeks assistance in performing procedures which require clarification or additional training.
- Maintains patient safety, comfort and cleanliness.
- Identifies patient and medical record with accuracy before administering care as evidenced by absence of documented errors related to misidentification.
- Consistently evaluates patient’s capacity to safeguard self prior to administering care and provides assistance as needed.
- Adheres to techniques outlined in Universal Precautions policy.
- Reports all necessary information and/or unusual occurrences in accordance with established policies and procedures.
- Completes incident reports immediately in all required in situations.
Consistently notifies appropriate immediate supervisor of unusual and/or urgent occurrences when they develop. - Reports actions of others which negatively impact patient care when appropriate.
- Serves productively in Hospital/Divisional Unit organization.
- Attends Staff meetings unless excused or off-duty as evidenced by review of attendance rosters and communication validation tool.
- Reviews posted information as evidenced by signatures on communication validation tool.
- Communicates outcomes of committee meetings to appropriate personnel.
- Provides input into annual Unit goals and objectives.
- Participates in staff development and educational activities. Demonstrates competency, knowledge and skills necessary to provide care to specific age-related populations (i.e., Neonates, Pediatrics, Adolescents, Adults, Geriatrics).
- Maintains BLS certification an annual basis.
- Attends all mandatory in-service updates on an annual basis as evidenced by review of attendance rosters and/or communication validation tool (such as fire/safety and infection control)
- Attends a minimum of four (4) continuing education programs on an annual basis as evidenced by review of attendance rosters.
Requirements:
- Graduate of an accredited Practical School of Nursing.
- Possession of a valid registration as a Licensed Practical Nurse in the State of New Jersey.
- BLS Certification required, as approved by the American Heart Association.
