Job Description
The Licensed Vocational Nurse (LVN) on our Community Health & Outreach team engages Native American patients in developing self-management health plans in addition to assisting with referrals, home/clinic/community visits, and specialized community health education programs/events. Programs may include chronic disease prevention, tobacco use prevention, senior health and wellness, injury prevention, and other community health promotion/disease prevention projects, activities and programs. Priorities of patient education include, but are not limited to: illness prevention, health promotion, individual, family and community needs assessment, promoting healthy goals, and coordinates community health programs and services under the direction of the Community Health and Outreach Director. The LVN performs routine technical nursing duties within the scope of the Nursing Practice Act and maintains standards consistent with the level of care set by Title 22.
ESSENTIAL JOB FUNCTIONS: The list that follows is not intended as a comprehensive list, but rather to provide a representative summary of the major duties and responsibilities. Incumbent(s) will be required to perform a variety of duties listed, including but not limited to:
- Provide care coordination services for an assigned caseload to support patients with health care needs through either phone consultations, home visits, clinic appointments and/or community contacts.
- Creates a patient-centered self-management plan that integrates lifestyle interventions to assist patient in successfully meeting improved health goals.
- Work in collaboration with all providers involved in the patient’s care to develop and facilitate attainment of identified self-management goals.
- Educate patients, families, and communities on the importance of preventative and wellness, health-promoting lifestyle interventions, such as
- immunizations,
- promotion of health screenings,
- smoking prevention and cessation,
- nutritional counseling,
- obesity reduction and prevention,
- increasing physical activity,
- substance use prevention/early intervention and harm reduction and
- promoting independence and skill development related to disease self-management and self-administration of medications.
- Oversees referral program, providing support to CHW/CHRs of those assigned to them, and maintains tracking system of all internal and external referrals.
- Facilitates and participates in program quality improvement projects, as well as Government Performance Results Act (GPRA) activities to improve program workflows and patient health outcomes.
- Perform screening tests (Blood Sugar, A1c, vital signs and measurements: height & weight) as per NVIH procedure.
- Network with community agencies to establish partnerships that align with the mission of NVIH to impact the health of the communities served.
- Seek additional funding sources through grants for program development within the departments.
- Provide guidance and coordination of patient services provided by Community Health Workers/Community Health Representatives in absence of Community Health/Outreach Director.

Experience and Skills
REQUIRED QUALIFICATIONS:
The following knowledge, skills, and abilities are required for an employee to successfully perform their duties.
- Current, active California Vocational Nurse license.
- Minimum six (6) months experience, including but not limited to medical / clinical setting. LVN and or two (2) years’ experience preferred.
- Effectively communicate in a clear, concise manner, including but not limited to public speaking
- Work independently under limited supervision, prioritizes work assignments, show excellent organizational skills, and works well under pressure of deadlines.
- Demonstrates understanding of age-related or specialty issues and developmental needs for each population served by adapting style, treatment and plan to each population.
- Ability to use electronic health records and maintain current, accurate medical records, charts and health plans. Familiarity with Microsoft applications, such as Outlook, Excel, Word, & Teams.
- Ability to travel and operate GSA vehicle for business purposes.
- Affiliation or experience in Trans-cultural nursing helpful

PHYSICAL ACTIVITIES REQUIRED TO PERFORM ESSENTIAL FUNCTIONS:
- Sitting / Mobility: Approximately 70% of time is spent working at a desk. Balance of time (approximately 30%) is spent moving around work areas.
- Communication: Ability to effectively communicate with co-workers, Board members, members of outside agencies, in person, by telephone, and by email.
- Vision: Ability to effectively use a computer screen and interpret printed materials, memos, and other appropriate paperwork.
- Lifting / Carrying: Ability to occasionally lift and/or move objects weighing no more than 25 pounds.
- Stooping / Kneeling: Ability to access files/stock supplies in low cabinets and shelves.
- Reaching / Handling: Ability to input information into computer systems and retrieve and work with appropriate paperwork, equipment, and supplies.
