JOB SUMMARY

The Intake RN will work closely with the Intake and Scheduling team to provide Intake and triage coverage for the Agency. The Intake RN will receive and provide clinical review for all incoming referrals for Home Health and Hospice. The primary focus of review will be to ensure referrals are processed in a timely manner, have a clinical/skilled need and are in line with all regulatory components. The Intake RN will also serve as a liaison between NCHHHA and referral sources.

Education and/or Experience:

- Graduate of an accredited nursing program
- 2-years recent experience as an RN in community health, medical/surgical or in an acute or skilled long term care setting preferred but not required (willing to train the right candidate)
- Possess a valid RN License issued by the State of New Hampshire
- Ability to read and interpret written documents
- Computer competency including the ability to retrieve information necessary to complete job responsibilities

- Excellent phone etiquette

- Able to work with minimum supervision
- Able to read, write and speak English
- Is physically and emotionally capable of performing the job requirements
- Licensed, insured ($100,000/$300,000 limits of liability) and reliable automobile
- Valid Driver’s license
- Able to satisfactorily meet all Post-Offer Pre-employment requirements (including
physical capacity exam, background checks, etc.)

Licenses/Insurance:

Access to operable, insured motor vehicle and valid driver’s license with acceptable record of safe driving practices.

ESSENTIAL FUNCTIONS:

- Responds to and/or triages calls in a timely manner and documents all telephone contacts, patient care observations and interventions.

- Receives and records accurate and thorough referral information required for services.

- Monitors all portals and platforms used for referral management for incoming referrals and orders.

- Works with referral sources to process urgent referrals and schedule them as soon as needed to meet patient/referral source needs.

- Creates charts accurately and timely. Inputs all information without error, including demographics, insurance, medications, intake note, Face to Face information etc.

- Records the outcome of all inquiries and referrals received and provides appropriate follow-up when an inquiry or initial referral cannot be serviced during office hours

- Communicates with the interdisciplinary team to ensure proper patient follow-up for complex patients or patients who may not be identified as being referred to the appropriate program

- Provides accurate information to customers making telephone inquiries regarding Home Care and, Hospice and other agency services.

- Assists with document management which includes but not limited to sending documents to Providers via the EMR and/or fax, receiving orders and uploading to the patient’s chart as appropriate.

- Performs audits as necessary under the direction of the Director of Clinical Services

- Assists with EMR training and implementation.

- Demonstrates good organizational skills and exceptional customer service skills

- Uses equipment and supplies in an appropriate and cost-effective manner

- Acknowledges that all information related to employees, clients and their families, and NCHHHA business practices is confidential

- Practices proper body mechanics and observes safety measures

- Adheres to all safety policies including reporting/ removing hazards immediately

- Demonstrates knowledge and compliance with infection control policies

- Follows instructions, responds to management direction

- Works cooperatively with other NCHHHA personnel

- Complies with NCHHHA’s Standard of Conduct

- Required completion of time sheets and other records in accordance with agency policy and State and Federal regulations
