The nurse navigator is responsible for collecting nursing assessment data for communication to the physician, the patient, the patient's family and the nursing staff for facilitation of the surgeon's assessment. Visit patients as directed by the surgeon for the purpose of collecting data through chart review, interview and observation. Obtain the history of the presenting illness to present to the surgeon. Review the patient's record and present information to the surgeon that pertains to the presenting illness. Record and document within the patient's medical chart including vital signs, laboratory findings and wound descriptions. Perform the duties as a nurse as needed when caring for the patient that is within the scope of practice for an RN or LPN within the State of Indiana. Record telephone orders from the surgeon on the medical record consistent with the hospital policy. This includes reading back the telephone orders and having the surgeon cosign the orders. Prepare the discharge summary with additional notation, completion, and signature performed by the surgeon. Provide patient and family education regarding diet, medications, disease information, exercise and activity level, discharge planning and follow up care as determined by the surgeon. Perform procedures consistent with hospital policy and procedures including but not limited to dressing changes, removal of sutures, and any other procedures that are within the nurse's qualifications and are requested by the surgeon. Be available to assist in the OR as needed after being adequately trained by the surgeon.

Requirements:

36 hours per week - First Shift

Monday through Friday from 7:00 am to 5:00 pm

Valid RN or LPN license in the State of Indiana required

Eligible for medical coverage on your first day of employment, all other benefits will be effective the 1st of the month following hire date!
