Job Summary and Qualifications
The sepsis coordinator is a RN who has 24 hour responsibility for the sepsis program. This person works in collaboration with the medical and hospital staff to provide multidisciplinary care to the septic patient. The Coordinator provides clinical, professional expertise for patients, families and staff members. The Coordinator facilitates development and refinement of the sepsis protocol to become the standard of care within the facility.
What you will do in this role:
- Educate hospital staff on the best practices of sepsis management.
- Develops and supports education to meet all requirements of sepsis management.
- Develops and recommends protocol/guidelines for best practice of the septic patient across the continuum of care.
- Oversees, analyzes, and trends data related to throughput of sepsis patients.
- Provides data to the Administrative leaders and/or other applicable hospital leadership to initiate change as appropriate, in order to improve processes and outcomes.
- Work with medical staff to assure continuity of care in the septic patients and to ensure quality outcomes occur.
- Provides organized care to the sepsis patient with a focus on health promotion and disease prevention.
- Works closely with Emergency Medical Services to assure optimal continuity of care from pre-hospital to hospital.
- Ensures metrics are in place to show continuous improvement in the care patients with sepsis.
- Acts as an internal hospital resource and externally in the community regarding education of the septic patient.
- Participates in identifying needs, planning and implementing educational programs for patients, families, community groups, and other designated healthcare facilities to improve outcomes.
- Represents the hospital at local, regional, or other applicable levels related to best practices for sepsis patients.
- Assures compliance with all TJC and other applicable regulatory agency guidelines.
- Demonstrates effective communication skills, interpersonal relationships, and team building skills.
- Assists with other duties as assigned within scope and practice of licensure.
- Assists with administering sepsis planning in collaboration with, all departments, management leadership and designated hospital committees.
- Sets high standards of performance goals and works diligently to achieve them.
- Coordinates quality improvement process to ensure high quality of care for the assigned population.
- Applies knowledge and skills related to growth and development over the life span with a focus on identifying the different needs of different age groups.
- Provides organized care to the patient with sepsis with a focus on health promotion and disease prevention. This includes but is not limited to entering orders and documenting in the medical record as appropriate.
- Ensures multidisciplinary rounds on sepsis patients are performed daily to identify missed opportunities with bundle compliance and to provide just in time education to frontline staff as appropriate.
- Gathers relevant information systematically as it relates to biological, physical and environmental factors that impact assigned patient population.
- Maintains current knowledge of hospital procedures that relate to management of patients with sepsis.
- Conveys a sense of urgency and drives to obtain closure on barriers for managing assigned patient population.
- Uses financial and quantitative information related to sepsis management issues and its impact on human health.
- Demonstrates knowledge in the area of sepsis care and is regarded as an expert in the area.
- Demonstrates understanding of issues relevant to sepsis management and its impact on the organization.
- Handles daily work challenges confidently and is willing to adjust to multiple demands and shift priorities when deemed necessary.
- Actively pursues learning and self-development; seeks feedback and modifies behavior when deemed appropriate.
- Acts as a resource to all employees for initiatives associated with the NPSGs.
- Participates in the development and maintenance of policies and procedures which provide for a programmatic framework for quality, performance improvement and regulatory readiness.
- Communicates effectively to coordinate activities with management leadership, healthcare workers and patients /families. Speaks clearly and expresses self well in group and or one on one conversation.
- Continues professional growth through the use of in-service educational programs, seminars, professional associations, and other self-directed learning activities related to care and management of sepsis.
- Utilizes organizational skills effectively in compiling and analyzing data.
- Is actively involved in community public health efforts, as appropriate.
- Creates an atmosphere in which timely and relevant information is expressed, as well as provides opportunities for individuals to convey understanding of concepts and be able to express ideas and opinions.
- Conducts assigned system/patient tracers throughout the organization and assigned units, reporting outcomes and implementing plans for improvement.
- Interacts with delegating physician in regard to general medical practice parameters for guiding care of the sepsis patient.
What qualifications you will need:
- Current BCLS, ACLS
- Ability to collect, interprets, and analyzes data.
- Excellent computer skills, including Microsoft Excel, PowerPoint, Word and Office.
- Ability to apply critical thinking skills for process improvement and decision making.
- Ability to interact as a speaker with good presentation skills.
