The care provided by our team allows people to pass with dignity in their own homes, in familiar surroundings, with those they love by their side. The RN Care Coordinator works with physicians and multidisciplinary team members to develop and manage a plan of care from admission to discharge for an assigned case load. This is a direct care position providing in home care to include symptom management, emotional and spiritual support, medication management, education and IDT oversight. The person needs to be able to work autonomously and as a team, have interpersonal and communication skills, and have a true passion and desire to care for end-of-life patients to ensure their needs are met.

What you can expect from this role
- Work/life balance | Full Time or Part Time Days (Mon-Fri 8 or 10 hour shifts) with rotating evening/overnight call and weekends
- Provide individualized care for patients from children to elderly
- Manageable case-loads (Daily visit requirements below state and national average)
- Pay for drive time
- Mileage reimbursement (Geographical Area: McCall, ID and it's surruonding areas)

Qualifications
- Education: Nursing Degree
- Experience: 2 years of RN experience; Hospice or Acute Care experience helpful
- Licenses/Certifications: Current RN License in the State of Practice. Current Basic Life Support (BLS) Certification Provider Certified through American Heart Association or American Red Cross
- Valid Driver's License and proof of Auto Insurance.
