NURSE MANAGER for our
MIDDLEBORO PROGRAM
40 HOURS- Schedule: 7a-3p Mon-Fri
$12k Sign On Bonus
(pay out schedule: $3k at 30 days, $3k at 6 months, $6k after 1 year)
$35.50 hour - (LPN/RN)
General Statement of Duties:
A lead member of the multi-disciplinary team. The Nurse Manager will case manage the health care needs of individuals living in a community setting; provide training to staff; communicate health care information to funders and other stakeholders.
Direct Care & Coordination of Medical Services Responsibilities:
- Identify health related needs, monitor health status and work with staff, physicians, and allied health professionals to formulate, coordinate and implement health related treatment plans and or protocols.
- Administer medications and treatments as prescribed, ensuring adherence to funders regulations, CARF, and DPH Medication Administration Program policies and regulations.
- Provide direct care and support to individuals served as needed, including ADL care, transportation, and community outings.
- Provide accurate, and timely health related updates on individuals served to stakeholders including families, funders, guardians, and BAMSI team members.
- Ensure medications, medical and safety supplies are available as needed.
- Coordinate hospital admissions and discharges.
- Utilize Positive Behavior Supports (PBS) practices, to create a thoughtful and supportive environment by ensuring activities, plans, and routines reflect the preferences, needs and goals of individuals.
Staff Management & Development:
- Assist with interviewing and hiring nurses within the program.
- Identify and provide training to both staff and individuals served in relevant areas (e.g., health, hygiene, safety, diet, etc.).
- Provide oversight of daily operations in absence of supervisor or designee, as requested.
- Collaborate with the Program Manager to provide guidance for direct care staff and nursing staff, including implementing the progressive discipline process and recognition of staff achievements.
- Supervise nursing staff.
Quality Management:
- Maintain medication books, logbooks, etc. to ensure program compliance with all regulations.
- Ensure safety standards and infection control procedures are followed.
- Complete accurate written reports, assessments, etc.
Staff Management & Development:
- Perform other related duties as needed or as assigned by supervisor or designee.
- Provide coverage at day habilitation programs or other residential programs as needed.
- Provide 24 hour on-call, on a rotating basis, to ensure nursing coverage, provide medical support, and access to resources.
- Rotate schedule to work holidays as needed.
Qualifications:
- Licensed Practical Nurse or Registered Nurse in the Commonwealth of Massachusetts
- 3 years of nursing experience required.
- Prior supervisory experience preferred.
- CPR certification required.
- Knowledge of the theories, methods, procedures, and practices of nursing care.
- Two years’ experience working with individuals with disabilities preferred.
- Must possess a valid driver's license in the state of residence and satisfactory driving record.
- Basic computer literacy, including the ability to record data, use electronic time and attendance software, and receive and send electronic mail, as needed, is required.
- Effective critical thinking, writing and communication skills.
- Ability to lift, transfer, push/pull, maneuver, and reposition 25 pounds.
- Ability to reach, bend, stand, sit and walk; perform fine motor activities.
- Work effectively in a moderate to occasionally loud work environment.
