Job Description

Assesses, plans, implements and evaluates the nursing care of residents within the skilled nursing setting. Responsible for ensuring continuity of care of the residents between shifts by providing direct care as well as supervising the care given by CNAs, LPNs/LVNs and supportive staff members. Participates in resident and family teaching. Maintains role as resident advocate with a focus on the facility’s mission.

6:00pm - 6:am with weekend rotation. Pay based on experience starting at $27.68

Responsibilities

Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the resident and family regarding pain management., Ability to interpret results of waived tests; takes appropriate action on waived tests results., Assesses residents on admission and at least quarterly, as well as when conditions change. Documents appropriately including Minimum Data Sets., Assesses the resident’s condition and nursing needs; sets goals; prescribes appropriate nursing action., Documents changes in resident’s condition and nursing care on progress notes and plan of care., Notifies physician of changes in resident’s condition and follows through until appropriate action is taken., otes and carries out physician’s orders in a timely manner., Performs all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors., Submits written documentation in timely, clear, concise and pertinent manner., Supports and maintains a culture of safety and quality

Requirements:
