Maple Farm Nursing Center offers an innovative model of short- and long-term skilled nursing within two warm and welcoming households situated on 66 acres in Akron, PA.

Job Title: RNAC

Department: Skilled Nursing

Status: Part Time Days, 16 hours per week

Salary: starting at $34/hr

What will I do in this role with Maple Farm?

As an RNAC, supporting the Garden Spot Communities' mission will include the following responsibilities:

Coordinate and oversee all functions of the MDS assessment process to assure compliance with federal and state requirements.
Schedule or oversee scheduling of all resident MDS assessments to assure timeliness.
Participate in tracking changes in residents' level of care to assure timely scheduling of MDS assessments.
Review all MDS assessments and related forms to certify timely completion by signing the MDS forms on the appropriate sections.
Examine residents to collect information related to the minimum data set assessment.
Review medical records and records related to residents' health status to collect information related to the minimum data set assessment.
Enter data into the computer program for minimum data set assessments.
Assume accountability and responsibility for accuracy of Minimum Data Set (MDS) assessment information.
Participate in the pre-admission process to determine potential residents' level of care
Review reports of timely completion of residents' MDS assessments and care plans and take follow-up action as necessary.
Participate in the Quality Assessment and Assurance Program, for the nursing department, in cooperation with the Quality Assurance Committee.

What do I need for RNAC with Maple Farm?

Registered by the Commonwealth of Pennsylvania as a licensed registered nurse.
Experience and understanding of the MDS process and its relationships to Medicare reimbursement and assessment process.
Ability to understand and utilize facility MDS software and computer skill.
