Job Description Summary:

Under the general supervision of the Director, and Clinical Coordinator, and responsible for providing hygienic care and assigned procedures for assigned patients based on age-specific needs. The employee is subject to call back and overtime as required by the hospital.

Responsibilities

Performs direct and indirect patient care responsibilities as assigned by the case manager or Clinical Coordinator.

Ensures patient safety while performing delegated tasks.

Performs related responsibilities as required or directed.

Provides emotional support to families of terminal patients.

Obtain vital signs as delegated.

Perform baths, linen changes, shampoos, shaves, mouth care, and nail care.

Assists patients with a transfer in the home as delegated and if included in the care plan.

Chart and turn in notes timely according to agency policy.

Maintain POSITIVE HELPFUL ATTITUDE.

Uses AIDET on a daily basis.

Communicate all pertinent information to the case manager.

Qualifications

EDUCATION: High school diploma or the equivalent required.

LICENSURE, REGISTRATION, CERTIFICATION: BLS certification must be obtained within the orientation period, and certification must be kept current while in this position. Must have a valid driver’s license and auto insurance.

EXPERIENCE: Current skills and knowledge obtained from the completion of an approved Nursing Assistant program or certification required.
