Redeemer Home Care and Hospice is seeking an experienced Hospice LPN to join our team in providing quality, compassionate care for our patients and their families throughout southeastern Pennsylvania. Apply today!

This position is eligible for a $3k sign on bonus. In addition, we offer comprehensive compensation and benefits for eligible employees, including:

WHY REDEEMER:

Hospice LPNs enjoy a flexible schedule, providing 1:1 direct care in patient homes with the support of an encouraging and experienced clinical team. You will teach patients how to manage their own disease processes in their own environment, focusing on quality care and evidenced based practice.

SUMMARY OF JOB

A Hospice Visiting LPN provides safe, holistic care to a population with multiple co-morbidities and advanced chronic illnesses under the guidance of the Hospice RN. The LPN functions as a member of the health care team and provides support to meet the needs of the patient, family, and community.

RECRUITMENT REQUIREMENTS

Minimum of 2-3 year acute or post-acute required; able to work weekend, holiday, and on-call hours; able to demonstrate professional competencies in a home care setting; able to efficiently utilize technology (cell phone, computer); able to travel in service area.

LICENSE AND REGULATORY REQUIREMENTS:Current State LPN License, Valid Driver’s License, Current CPR Certification
