Roatating schedule, week one 40 hours, week two 32 hours
*This position is at our skilled nursing facility, Ingraham Manor located at 400 North Main Street, Bristol, CT.
Provides direct clinical care services to clients under the direction of Director / Charge Nurse.
Essential Job Responsibilities:
Effectively participates in the clinical care of clients in support of the nursing function
- Conducts vitals on all clients monthly and as needed
- Accurately collects the readings on all vitals and reports any abnormal readings to the Charge Nurse immediately.
- Demonstrates effective skills in blood pressure readings, temperature readings, and cardio-respiratory readings by accurately taking all readings.
- Effectively measures weight and height of a resident as needed.
- Collects urine and/or stool specimens as required.
- Changes ostomy bags as required.
- Conducts admission vitals, weights and photos
- Timely and accurately documents all pertinent information regarding clients, consistent with facility policy.
Actively participates in the delivery of care services to clients in accordance with the facility standards.
- Administers personal hygiene to clients, including but not limited to bathing, shampoos, shaving, cleaning and trimming nails, mouth care, and denture care.
- Assists clients with dressing and undressing in an appropriate time frame.
- Renders incontinent care as required.
- Toilets clients in accordance with facility policies and as needed.
- Demonstrates a knowledge of and is able to implement feeding of clients.
- Identifies needs for assistive devices in the feeding of clients.
- Safe service of dietary trays in a timely manner to maintain appropriate temperature of all foods
- Transports clients as required.
Ensures residents rights are maintained in the course of the delivery of care.
- Always wears employee identification badge consistent with facility policy. Identifies self and purpose and intentions to the client and any family present.
- Ensures there is privacy for the client during times when delivering care.
- Respects the private space of a client.
- The clients are well groomed in a manner appropriate to their age and culture.
- Client is assisted in dressing in a manner that promotes individuality and personal preferences.
- Client is always spoken to in a respectful manner.
- Client is listened to carefully and concerns addressed on a timely basis.
- Client is addressed with the name of choice.
- Emergency bells are answered immediately consistent with facility policy.
- Client confidentiality is always maintained.
- Clients dignity is maintained through the discreet discussion of the clients status with family and other staff i.e.: bowel and bladder habits, etc.
- Clients records are kept secure and documentation is done in such a manner so as to ensure the confidentiality of the client record.
Environmental care promotes a health and orderly environment for the community of residents within the facility.
- A clean and orderly work environment is maintained at all times.
- Universal precautions are taken in the delivery of care and maintenance of the work area.
- Disposable gloves, appropriate protective clothing where required, use of Biohazard waste disposal, disposal of sharps and no cross-contamination via linen or equipment occurs consistent with facility policy.
- Isolation Technique is followed where required.
- Clients personal space is kept neat and orderly.
Resident safety is always assured.
- Monitoring alarms are checked daily for functioning according to the facility policy.
- Selected clients have monitoring alarms applied according to the facility policy.
- Clients are transferred safely and in accordance with the facility guidelines.
- Clients are properly restrained in W/C for transport according to facility policy.
- Proper supports are used when needed.
- Water used for bathing of client is of proper temperature so as to be comfortable without scalding.
- Assistive devices in the transfer or ambulation of a client are used as required.
- Changes in the clients status that may indicate a need for an assistive device is promptly reported to the charge nurse.
- The floor is kept clean and dry and free from any spills or other falling hazards.
- Observes all fire safety and related rules and regulations. Is proactive in helping ensure the safety of residents, visitors, and co-workers.

Qualifications
Educational / Minimum Requirements: High School Graduate or GED or enrolled in high school. Graduate of CNA long-term care certified training program.
State/Federal Mandated Licensure or Certification Requirements: Current CT CNA registration.
Bristol Hospital Mandated Educational Requirements: General orientation at time of hire. Fire/Safety/Infection Control annually. Other programs as mandated by Ingraham Manor and Ingraham Adult Day Center.
Special Requirements: Must demonstrate patience and kindness toward clients and families. Must enjoy working with geriatric population. Must possess good observation and communication skills.
Physical Requirements: Must be able to perform all aspects of nursing care. Must be able to lift a minimum of 50lbs. Must be able to endure prolonged periods of extended walking, standing and stooping. Moderate periods of sitting, reaching and bending. Occasional crouching, kneeling and balancing. Vision, hearing, smell and tactile sensation corrected or accommodated to normal.
Work Environment: Regular contact with clients, families, body substances, and hazardous chemicals. State regulated temperature of 75 degrees; varied lighting; moderate noise. Use of various types of lifting devices.
Cognitive Requirements: Must be able to read, write and follow oral and written instructions.
